There must be the wrong id of reward.
莎莱兰佳的手镯 ( item=186654 ) that's the item and 莎莱兰佳之手 ( spell=354355 ) is the summon mount spell and the item with id 354355 just doesn't exist

All "Hand" Mounts

Gold: 耐迦尼赫玛特之手, obtained from 耐迦尼赫玛特的掌控戒指. You have to find five parts and combine them to complete 给你露一手. See the top comment on 耐迦尼赫玛特的掌控戒指 for details!
Silver: 雷什基加尔之手, obtained from 雷什基加尔之手. The October 2024 Trading Post offered it for 750 Tender.
Warm-tone Silver: 芭美兹拉之手, obtained from 芭美兹拉之链. It's a low drop chance from 折磨者宝箱, which itself is a once-weekly drop from the Tormentors that spawn every two hours in the Maw.
Cool-tone Silver: 赫雷斯迪莫拉克之手, obtained from 赫雷斯迪莫拉克的护腕. It's a reward for doing all the Sanctum of Domination achievements — 统御团队的荣耀.
Black (this one): 莎莱兰佳之手, obtained from 莎莱兰佳的手镯. It's a reward for killing every Tormentor — 打破锁链.

Impossible to get now due to Blizz never fixing the bug where tormentors never spawn. It's been years.

Here's something you can copy and paste into the Bug Report in-game. You could also submit it as a Suggestion at the same time:

The Tormentors of Torghast event is still broken in the Maw after many months. This prevents completion of the achievements Minions of the Cold Dark and, in turn, Breaking the Chains (the Maw / Korthia meta achievement).

If you are unable to repair events in the Maw (the "Army of the Jailer" event is also broken) then please remove the Minions of the Cold Dark from the meta achievement Breaking the Chains.

THIS MOUNT IS NOW UNOBTAINABLE ! THE ACHIEVEMENT IS BUGGED !
Do not waste your time ! You wont be able to complete it to get the mount It has been bugged now for over 12 months. (and not likely to be fixed any time soon )
The part that you cant finish is the Minions of the cold dark. The tormentors of Torghast special encounter in the maw is bugged the progress bar does not appear so you cant start it or finish it. I wasted many hrs before if discovered this hopefully this will save you that pain.

The event is working again. Just killed a tormentor 3min ago. The progress bar fills up and the tormentor spawns. So the Achievment is obtainable again
