To get the plans you must give one Sulfuron Ignot (Epic item that drops in Molten Core) to the Thorium Brotherhood NPC in the top layer of the bar in BRD (Black Rock Depths).

I managed to pick mine up a few weeks before the expansion for 20g. Id think now days they are very hard to get hold of as most guilds have stopped raiding Molten Core.

Sulfuron Ingot are the hardest items to obtain to craft just this hammer. They have about a 22% drop rate from Golemagg so it'll be a while before you can farm it unless you go more than once a week using alts or another method.

Blood of the Mountain can be easy/annoying depending on how you look at it. If you have patience just keep mining Dark Iron Deposits in Burning Steppes and you'll eventually get enough.

Essence of Fire are easy, if you farm the other mats you should just have enough when you have the harder mats such as the Sulfuron Ingot and Blood of the Mountain, if not AH is a good friend. If you really want to just get this then head to Un'goro Crater and farm the fire elementals at Fire Plumb Ridge (center of the map).

Lava Core/Fiery Core may be a pain for some. MC trash can be cleared with a few people so if you can go in with friends and clear all the trash you should get enough or just short in one run. If not clear the trash 2-3 times and you'll definitely have enough.

Arcanite Bar are easy, just farm a bunch of thorium veins. You should get some Arcane Crystals from the veins and if you don't have enough they should be cheap on your AH.

Assuming you going for the achievement Sulfuras, Hand of Ragnaros which most are, the Eye of Sulfuras is the hardest without a doubt the hardest item to obtain. It drops from Ragnaros and has a 3-5% drop rate.

I started collecting the mats when I got the Eye. The Sulfuron Ingots are definately hard to find, and the Blood of the Mountain is also very rare.
The Lava and Fiery Cores are actually quite easy to get by farming the lava packs.

Also for the Arcanite don't forget that there's no cooldown on the transmutation anymore ! (I made them one by one at first -_-' )

This does still have to be made at the Black Anvil still, as of Cata.

Once you've got all of your materials and the plans learned, head to BRD and take the left fork until you reach the Dark Iron Highway. Instead of going left and killing Bael'garr, you want to head right and go through a big door, continuing down a corridor with some fire elementals, until you reach the anvil. The boss has about 60k health, so you may want to bring a friend if you're lower level. I was able to go in as lvl 60 with my lvl 71 friend and I could walk around the trash without a problem, and the boss fight went fine. At higher levels, 70+ or so, you should be able to solo the boss easily.

Once you've got your hammer made, all you have left to do is farm Ragnaros. The hard part is over! :)

edit: alternatively, you can use the mole machine and port to just above the anvil, and jump down. I haven't done this myself though, so I don't know if you can get up out of the lava or not.

Can this weapon be used for transmog, or does it come under the Legendary restriction?

Sulfuron Hammer still needs to be crafted at the Black Anvil, as of 5.4.8.

The Black Anvil is what 伊森迪奧斯領主 circles in BRD, but do not get it confused with the Black Forge, which is where you (still) go to smelt Dark Iron Bars, near the entrance to Molten Core.

Quickest route for high level players: Enter BRD. Take the Abandoned Mole Machine "Just past the Grim Guzzler" & jump down. Aim the jump so you don't land in the lava. Kill Incendius, craft your crafty things.

Quickest route for low level players: If you have a fall damage mitigation spell (緩落術 for Mages, etc.), you can use the high-level option. But be careful, there are two elite level-52 熾熱火焰衛士 where you'd jump down. Else: Enter BRD. Hang a left through one Shadowforge Gate, go straight until you start running into Anvilrage mobs. This is the "Dark Iron Highway" - go right (away from Bael'Gar) through the BIG open doors. Keep going until you see Lord Incendius. Kill Incendius, craft your crafty things.

As 3/10/2024 DF 10.2.5 You still need to go to the Black Anvil in BRD. The tool tip in the Blacksmithing window states "This recipe requires you to be near a special crafting station. These can usually be found in the crafting areas of major cities" - this in incorrect in this instance. I did try the anvil in Orgrimmar, no dice.
The directions from albinoette, copied below, were perfect.
