Part of the Shaman Tier 4 set (http://www.wowhead.com/?itemset=632), token drops form the last boss in Karazan. Taken by itself, not much better than Shamanistic Helm of Second sight (http://www.wowhead.com/?item=31107), but don't forget the set bonus. Even with the clearcasting rule, the 4 piece bonus is not consumed by free spells, and makes healing cheaper.

Same as what Deshank stated, plus with the buff from the 2.1 patch to the totem you can buy with badges (http://www.wowhead.com/?item=29389) lowering lightning bolt from 297 to 267... the 4 piece set bonus ... this is basically giving you a slightly weaker version of the talent Elemental focus, the way it is currently (pre 2.1)

This is part of Tier 4 Raid set (lvl 70) . You can obtain this item from 阿羅迪斯‧日刃 if you are friendly with 占卜者 , or from 阿蘇爾 whitch is member of 奧多爾. You need appropriate token to purchase this item.
