The thread is in the small room to the left as you enter. The cleaver is at the end by the lab equipment. The "Slab" is made of dark granite and is near the entrance, overlooking the lab equipment. You can save time by jumping down, getting the cleaver then clearning back to the entrance to use the items.

Ok there is only TWO item to loot, so don't do like I did and look for the 3rd item because there isn't the slab is where you summon that thing.

1. You go in the cave at 34, 68
2. go down a bit and turn left, head into the room and get your Thread.
3. get out of the room and jump down towards the lab
4. get up to the lab and get your cleaver.
5. kill your way back up towards the entrance and use Crusader Oaklin's Remains that's in your bag at the platform looking thing which is whereabouts you jumped towards the lab earlier.

This Draenei looks in really good shape once the quest is over.
Even though he was torn apart by a frost wyrm, his body parts used to make abominations and sewn back together by someone who probably doesn't have a great knowledge of anatomy.

EDIT: In response to the replies to my comment, I didn't mean to imply that you needed a group for the quest. Was just stating that "put Humpty together again" and that the quest is called "Putting Olakin Back Together Again" made it sound liek that Blizzard got the idea/name of the quest from Humpty Dumpty :P

I don't know about you, you, but my DK has killed ~100k creatures (and that is very little compared to some people...) in his journeys. When you hack stuff to pieces long enough you're bound to pick up thing or two about anatomy...

Lol, this is daft, when I did this quest I was expecting him to be returned to life (after being ripped apart and used to make abominations) to at least come back as an undead draenei, but no, he comes back perfectly fine, not even a scar on him, and he just walks away like he just cut his finger or something.

Apparently Olakin never had a head. Only legs, arms, and a torso.

I just finished another Icecrown quest in which even Alexstrasza, Queen of the Dragons and the Naaru could not save a wounded warrior from the plague and let him live again in Azeroth, and yet this guy, who is dragged away by a bone dragon, torn to pieces, and his parts put into abominations who I can only assume are teeming with the plague, is easily reassembled by myself with some damn thread.

What is going on here?

Yes, go to the slab, click on bag of remains in your invintory.

To get these you go back into the tunnel where you freed patches to kill the doctor. A short way into the tunnel there is a room on the left, this is where you will find the spool of thread. You will also see the slab on the opposite side of the tunnel from the room. The cleaver is located in the *workshop* located at the end of the tunnel and can be seen from the slab.

The "large slab" is located at 36,67. The spool of thread is located at 35,66 and the cleaver is located at 37,68.

Putting Olakin Back Together Again

Obtain a Spool of Thread and the Doctor's Cleaver, then take them to the Sanctum of Reanimation Slab and use Olakin's remains to attempt to reanimate the crusader. Return to Darkrider Arly at Blackwatch when this task is complete.

We can't afford to waste any time putting Olakin back together. Fortunately, the Scourge has provided a perfect place for you to stitch him back together.

Near the Fleshwerks' flesh giant lab is the Sanctum of Reanimation. Search the area for a spool of thread and the doctor's cleaver. Then, take Olakin's remains to the large slab in the middle of the sanctum and get to work.
