『扭曲監視者』克里達斯 drops colored engineering monocles that can be used by characters who do not have the profession: 紫水晶眼魔之眼, 翡翠眼魔之眼, 燃燒眼魔之眼, 赤紅眼魔之眼.

For Cloth wearers:
赫瑪西恩 can drop 龍織衣飾, a black vest with a unique model (as it's a vest and not a robe), part of the Gossamer Regalia (Recolor).

狂野的米克索格 can drop 惡魔編織衣飾, a red vest with a unique model (as it's a vest and not a robe), part of the Wizard Regalia (Recolor).

弗拉肯‧厄語者 can drop 軍團護盔, a red helm with a unique model.

首長瑪庫隆 can drop 虛空鋼鐵護手, a purple glove with a unique model similar to those used by the 精英亡靈守衛 of Undercity.

弗拉肯‧厄語者 can drop 軍團頭巾, a chain helm with a unique model.

泥沼潛伏者 can drop 泥沼肩鎧, a green shoulder with a unique model that matches well with coveted transmog sets such as Jade Plate.

During the quest 暗影世界 (更多暗影之塵 after completing it once), you need to kill arakkoa in Skettis for 暗影之塵. Collecting 6 will give you an 暗影藥劑.

The 暗影藥劑 allows you to see Time-Lost, ghostly versions of arakkoas, which will drop 時光流逝卷軸.

You use 時光流逝卷軸 on the smaller summoning circles around Skettis, to summon one of four mini-bosses (you can choose which one you want to defeat): 女獵手吉札拉, 黑暗尖叫者阿卡萊, 凱羅格 and 『風怒者』瓦奇茲.
You need 10 時光流逝卷軸 to summon each mini-boss, needing a total of 40 scrolls to summon each one of them once.

Wave 3 - 貝許爾管制者s and 貝許爾計算者s spawn.

Wave 4 - 偉大的收集者 spawns and becomes attackable after you kill enough NPCs.

You forgot to mention about that rare that has a ridicularly long spawn time at Shadowmoon Valley, near the Black Temple, a big robot-like with Doom in its name, that they say can drop some rare mogs too. Don't know if there are other World Boses like it, like Kazzak in Hellfire peninsula too. Some people may know them too but they are so rare... might be even rare than those for the achievement.
