Feels like 50% to explode, but for me it was 37.5% chance to explode. Tall Mushrooms have nothing to do with it. Questie and Track Herbs are your friends here.

I gathered mine today.
I did ONLY find them on top of mushrooms.
I have 390/390 herbalism skill (tauren druid), although I am sure you don't need any herbalism skill to gather them, I attribute the fact that I had NONE of them explode on me, to my high herbalism skill.
I did make sure to point my back to the middle of the mushroom in case of knockback, and approached them carefully.
I also made sure to click at the base of the stem, but I tested on the last one clicking the top and still no explosions.
SO I am pretty sure I avoided the explosions from having high herbalism skill.

These are quest items looted from "containers" which look like herbs. You can loot them without Herbalism, and you cannot buy them in AH.

They are located on top of the the giant mushrooms - see Sipder2's locations or follow Questie markers.

When looted, they have a chance to explode a throw you off the mushroom. The chance was 3/13 for me.
