This is my transmog set. Took some time to get it, as I had to go back and do quests that luckily I had never completed. It's truly unique on my server, and I receive many compliments.

Rohan's Mask or Lilly's Bandana make for a nice match to the studded leather models featured in this set. Other helmets surely go well with this set, however I've become attached to this particular look.

Soul Eater Shoulderguard or Crested Shoulderguard or Bael Modan Monnion or Defiant Spire Shoulderguard among others available to Horde only. These are only available via quests but fortunately there are several sources. Chances are good you've not completed all of them and can still find one for yourself.

Cloak of War or Shroud of Dark Memories and many others that drop in Cataclysm reg & hero dungeons, have the same model.

Acid-Eaten Vest or Insect Vestments The first is available by quest. However the Insect Vestments can be farmed in Heroic Azjol-Nerub.

Rough Climber's Grips are the only gloves that match the robe/vestments EXACTLY! I've not found any other skin with such a perfect match. Unfortunately for some it's only available by questing in The Storm Peaks of Northrend. If you've completed this lengthy quest-line and disposed of the reward long ago, then you're out of luck. It's truly one of a kind. The quest line is faction neutral and begins with “They Took Our Men” and ultimately ends with "The Reckoning". (A Quest Tree can be found in this comment thread.)
EDIT: I found that these gloves of a different model but same skin exist. Gloves of Northern Lordaeron So you have two options with this color set.

Arctic Belt is easiest to obtain as it's a basic Leatherworking craft item. Others with same model are available via quest and drops.

Black Chitinguard Boots are the closest match I've found and as only the toe & instep of the boot is visible under the robe/vestments its studded leather model is perfect in my opinion.
EDIT: I found a better pair of boots that are available by quest to both horde and ally. Maloof's Spare Boots can be obtained via the quest “Two If By Boat”. It's the 3rd quest in the series that leads out of Feralas and into Thousand Needles. Once again, as only the toe & instep of the boot is visible under the robe it's coloring is a spot on match for this armor set. Treads of Banshee Bells & Lizard Skin Boots are other options that appear to be faction neutral. A Horde only verision of this boot is also available.
Failing all that you can try one more.. The quest reward for "The Burlap Grind" quest in Pandaria/Kun-Lai Summit. As you can see it's also a pretty good match.

Disregard any shirt, wrist and leg items for this transmogrification set. No model for those slots will be visible.

You'll find many weapons to compliment this set, however here are couple of favorites for my Druid:
Unknown Archaeologist's Hammer w/ Mistyreed Torch is my current equipment.
Tremendous Tankard O' Terror w/ Petrified Camel Haunch is fun!
Crystal Woodstaff
Stonesplinter Staff
Bloodfire Greatstaff
