I decided to buy this on an alt and I started to get Centaur Hunting Trophy from kills (appears to just be from elites/rares). I did not buy the item on my main and I haven't gotten any such trophies from any elites/rares.
So, from purely anecdotal evidence (i.e. it could just be a huge coincidence/RNG that I got these on my alt and not my main), it appears this item enables - or perhaps increases the chance for - the trophy drops from kills.

Sold by Farrier Roscha at Timberstep Outpost at the very NE edge of Ohn'ahran Plains (just as you exit Waking Shores) for 100G (80G in PTR but 100G in Live). Farrier Roscha's coords are 84.4 25.0. She also gives you a local quest.

This item actually does seem to work for random item upgrades acquired from quests!

My buddy and I were questing together and I got significantly more item upgrades, blues and epics while he didn't. He asked me how am I so lucky! And I realized I literally had the Lucky Horseshoe buff because of the item. So it seems like it does have some effect!

Anecdotal, maybe.
For 4 levels, I got maybe 2 upgraded quest items.
After getting this item, withing 5 quests I had two upgrades, one Rare one Epic

I feel like this item is a reference to another horseshoe with unclear effects from Lord of the Rings Online. https://lotro-wiki.com/index.php/Erebrandir%27s_Horseshoe

It took years before any effects of Erebrandir's Horseshoe were actually discovered but some people would always keep it equipped, not knowing what exactly it did.

This item adds another 'permanent buff', so if you're a collector of buffs (like the haunted memento, quel'dalar and others), it might be worth getting it for another extra slot.

Maybe i'm imagining things, but after having bought this, I feel like I constantly get epic and rare quest upgrades. Maybe they just changed the numbers for this stuff in dragonflight, maybe its the horseshoe, maybe it's just me? I don't know, but I do indeed feel lucky.

Keeping this thing in my bag, I've been getting lots of Rare and Epic upgrades to my quest/world quest reward gear. My BF does not have this lucky horseshoe, and he's getting only 1 or 2 upgrades a day, while I'm getting almost every item upgraded. Could be the placebo effect, but I really think this works. BF refused to buy it because he said it was just another one of Griftah's scams, but I figured for 100g, it's worth a try. Needless to say. MY iLevel is now 25 points higher than BF's and he went and bought a horseshoe today. LOL

After I bought this, genuinely did seem to start getting more gear drops and upgrades. Placebo? Maybe... but for the price, in this expansion, I will take it anyway if it is.

...which is why I went to buy one on another character. And it isn't there. It hasn't been there multiple times I've checked with this char. Haven't tested a third yet, but has anyone else run into the item not being available? In my case I own it on one char (who is 70 but has only done some quests) and have finished the story quest on the one I'm now checking with )who is also 70).

It appears that this item prevents appearance changing effects. I picked up this item while levelling, very early in the expansion, and all expansion long, I haven't been able to swap appearances using Reflecting Prism. Then, a few days ago, I put the Horseshoe in the bank, and ever since, I've been swapping appearances as I please.

well, two hours farming i get nothing, but when i was in a Quest Zones i got blues, like 3 in a day
it seems working

and i was playing with my another char without horseshoe, i got zero blues

its working guys, in a Quest Zones i highly recommend
