This item is part of the summoning process of the rare "Fruitface". Inside the Barrows of Reverie you talk with the dryad Mylune. She will give you the buff Mylune's Blessing. Once you have the buff, loot Missing Fruit around the Barrow until a Thieving Podling appears. Keep dpsing and following the Podling until he jumps into the water. After a wave of more Podlings, the rare will appear.

You can find Mylune by entering a cave in Emerald Dream at /way Emerald Dream 63.5, 71.7 Cave entrance.
Then go straight to /way Barrows of Reverie 60.2, 68.8 Mylune.
When you speak to her, she will offer a dialogue for you:

I'm so happy to see you here. Funkowah!
I left out some fruit for my tiny friends here, and someone stole it!
It's a great mystery that needs to be solved! With you here, I know we can find the missing fruit.

After you accept to help, you will receive Bénédiction de Mylune buff.
Then walk around and pick up Fruits manquants from the floor until Boguelin voleur spawns.
Attack and follow him until it explodes into several Boguelin espiègle.
After killing the podlings, Face-de-fruit spawns at /way Barrows of Reverie 53.9, 34.2 Fruitface.
