For anyone passing by without the knowledge - this achievement will be removed at the launch of Shadowlands pre-patch as stated by Blizzard, making the title and achievement permanently unavailable for people to aquire.

"One of the more challenging achievements to arise in Visions of N’Zoth—Mad World—will no longer be acquirable once the pre-patch goes live. If you successfully earn the Feat of Strength, you’ll be granted a fitting title, “the Faceless One.”"

For anyone else wondering why this Title did not unlock after completing 8 mask solo runs in both SW and Org, you have to run both with 5 masks exactly for this achievement.

Seems like this title is not available anymore (again). I wish there was a lot more transparency with all the FOMO Blizzard puts into this game. I spent so long grinding for this only to not get the title at all. Such a waste.

To begin, I would like to sincerely apologize for the inconvenience you're experiencing and for the delay in our response.

Regarding your concern, I understand you’re looking for clarity on whether the “Mad World” Feat of Strength and the Faceless One title are still obtainable, and if your recent solo 5-mask Stormwind run should have met the requirements.

This particular achievement and title were originally tied to completing both Stormwind and Orgrimmar Horrific Visions with all five masks active during their original availability. In the current Revisited Horrific Visions version, the exact unlock conditions may differ, and some past rewards, including Mad World and its title, may no longer be obtainable. Because this content and its rewards are no longer part of the active competitive progression, we’re unable to confirm eligibility or verify progress directly in-game. However, If you’ve already met all known requirements and the title still did not unlock, this could be due to the achievement being retired, or it may be a case where additional conditions are still required. The best next step would be to submit an in-game Bug Report so our Dev team can review your run, here I will provide you the link: https://us.battle.net/support/en/article/000015043 . While we can’t directly grant the title, any confirmed issues can be investigated and addressed in future updates.

EDIT2:

Both me and Blizzard were wrong. The title is still in the game and I earned it on August 20th 2025 after realm reset at 07:17, using 5 masks (Burned Bridge, Dark Imagination, Long Night, Pained and Vengeance). There is hope!
