In a cave to the west of Obsidian Citadel in Waking Shores. Doesn't seem to do anything? There's a repair action and an item near by that can also be clicked.

There are also 2 items left and right on some little braziers that give a 15 min buff each. Twilight Aegis (All stats increased by 5% Max Health decreased by 10%) and Fate Barrier (Chance when hit to gain an absorb shield) - they dissapeared after clicking on them. I have tried to repair this as engineer and nothing happened. Maybe it is working as Dim Forge where you craft a Molten Alloy and then you can loot something! We must repair this!

Seems like it should be an engineering quest. But with level 82 engineering, clicking on it doesn't successfully repair it.
Maybe you need 100 engineering? Or there's some secret unlock.

The location is in the Waking Shores at 34.63, 46.67 inside the cave. Cave entrance is at 35.80, 47.96. I still don't know what the Broken Waygate is for either and I'm an engineer. Hoping to find out through further exploration!

There is now also one within the Zskera Vault, still not sure how to use it.

I finally cracked the code (I don't know if this was blizzards intent all along?). There is an item you get in the Zskera vault called Neltharion's Toolkit that is used to repair a broken waygate in the vault which immediately made me think of this one. I went over with the item in my bags and was able to successfully repair the waygate and use it. It teleports you to a secret Zskera Vaults room (one that I couldn't get to despite opening all the doors in the normal vault) and has a Grand Cache upon entry. It only contained a few dormant primordial fragments, some elemental overflow, and a Renewed Proto-Drake: Antlers upgrade which is kind of disappointing. Glad to finally have an answer though.

Also, as an aside, in the vault there are broken eggs and a full dragon skeleton. The eggs display "Broken Egg" when you hover over them but the skeleton doesn't display anything.

CAUTION: The 坏掉的界门 inside the Vault in Forbidden Reach will NOT portal you to a secret room. You have to repair the 坏掉的界门 with the 奈萨里奥的工具套件 that is located in Waking Shores in the cave at /way 34.63 46.68
Click that 坏掉的界门 a couple time and it will transport you to a secret room with a single chest & spider. When you exit that secret room, it will put you outside the Forbidden Reach Vault.

GENERAL
Anyone can loot - is not profession based.
Toolkit required to repair Waygate.
Toolkit and Waygate can spawn at different times.
Toolkit will show up even if you have one in bank, but you wont be able to loot it, it will get mailed to you and disappear.

WORKAROUND
Not sure if this will work for everyone, but I travelled to Dealer Vexil in the Waking Shores ( /way 35, 48 Cave Entrance )
Right next to him was a portal that was active. I clicked it and it teleported me to the last room within the Vault - with no toolkit needed.
