Previously, each boss had a set of uniquely-named loot. Now, multiple bosses can drop loot of the same name, but different bosses are assigned unique item level versions of the items. For example, Bonemaw always drops the ilvl 615 version of Goldsteel Belt on Normal, Warlord Zaela always drops the ilvl 630 version on Heroic, and Vigilant Kaathar always drops the ilvl 570 version on Normal as Auchindoun is a lower level than Shadowmoon Burial Grounds.

Another change is that loot tables for each boss appear much larger than before. Instead of a boss dropping a belt, there is a chance that a boss will drop one of five belts for a particular armor type. These belts vary in the types of secondary stat combos they have--the combos are set in advance and are not random.

With Personal loot (default), you will always get an item from the final boss.

With Need before Greed active, you will get 3 items to be shared among the group.

Finally, Normal-mode dungeon gear has the chance to have a tertiary stat.

When you build your Garrison and hit 98, you will see Khadgar's Servant wandering around giving your Garrison denizens tours. He will have an orange ! over his head and will offer you your first quest for your Legendary ring, Call of the Archmage.

This quest requires you to travel to Talador to see Archmage Khadgar in his tower in Zangarra. If you had quested through the zone while leveling, you can fly directly there on flight path, otherwise you can simply travel there through the zone.

After speaking to Archmage Khadgar here, he will offer you Spires of the Betrayer which requires you to go to Skyreach to retrieve a quest item: Pure Solium Band. This quest can be completed on Normal or Heroic but it can not be completed on Challenge Mode. The quest item drops from the last boss of the instance, High Sage Viryx. This quest can be shared. Return to Khadgar in Zangarra. Hand in the quest and accept the next quest from him, Khadgar's Task. Speak to Khadgar, and receive your ring. Your reward will be based on the loot spec you have selected when you hand in the quest. The rings are epic and are ilvl 640: Solium Band of Mending, Solium Band of Might, Solium Band of Dexterity, Solium Band of Wisdom, Solium Band of Endurance. From here on, you can buy extra rings for your other specs from Zooti Fizzlefury, located just outside the Archmage's tower.

To me, this is one of the most important/helpful guides to have around on a website like this. It can be rather tricky to reach the ilevel necessary to enter a raid, or just below that: heroics, and the standard advice to run normals (or do PvP.... brrr) has become outdated in this expansion with so many different ways to get gear.
This way I can easily see where I could go to get upgrades for specific pieces of gear - without having to drudge through the item finder, keep changing filters to get all possible results, open a dozen tabs, and then still miss out on a few possibilities.

Especially the tip regarding the Dwarven Bunker/War Mill is one to take to heart: make sure you save the Nagrand quests (and preferably rares/treasures too) till last, get yourself said building, and then do the quests/rares/treasures. And once you're done with that, you could always choose to tear down the building and replace it with something more useful to your character.

There are also pieces of gear that come from Garrison missions. The first set of follower missions at (follower) level 100 can yield 615 gear (622 if it comes out "war forged").

While you can't control which missions you get, it is another possible option when gearing up (I picked up a trinket, shoulders, gloves and boots all through Garrisons - the gloves were war forged).
