Drops from Onyxia (25 man) raid. An ungraded version of the Netherwind Crown that originally belonged to the Mage Tier 2 set in Onyxia's loot table. There is another version of this lvl 80 item droped in the 10 man with slightly lower stats.

It is important to note that for those collecting the Tier 2 set from BWL for fun, the new Tier 2 style helms definitely do not count towards the set. Saw a lot of questions on the forums, just thought I would clarify here. :)

Update: All Tier 2 Helms have been moved to Nef's loot table. So Nef (Main/last boss in Blackwing Lair aka BWL) now drops both the Tier 2 chest and helm items. Source

The sad part of these new awesome helms is that for classes like mages that have great t8 2 set bonuses have a hard time breaking up the set for these if they earned t8 via emblems.

My suggestion is keep this helm at hand for when you get a chest or maybe a diff piece of t8 w/e comes first, its a HUGE upgrade. Just not big enough to make breaking the 2set worth it.

But for those who are geared mages running ToGC this most likely isnt a problem.

LOL, I was lucky one yesturday. Got this item + head of Onyxia + Red Gem. Also before that I won this same lvl 232 helmet form Onyxia 10, but after won this gifted that one to another mage.

Suprisingly good for deep fire PvP specs. High hit to help you reach the cap (124), very high crit, no wasted stats really. Even the 10 shadow and frost resistance is sort of a big deal.
