This is a
must
have for any 2h spec. This glyph leads to a huge DPS increase when combined with the Epidemic talent. For example, a 2h Frost DPS spec will have a 5 Obliterate rotation once the diseases are in place.
Dual Wield specs will most likely have little use for this glyph though. They tend to have very short rotations already but who knows what people will come up with :)

I'm still confused about this, even after Razmortis' efforts, but I gather that in order to refresh diseases on secondary enemies in range using this glyph, we have to hit with Pestilence twice, first to refresh the diseases on the target, and second to spread the refreshed diseases?
I'm concerned about this cos by being so it becomes more difficult for Unholy tanks to refresh DnD and still keep diseases on all targets. I'll try to demonstrate with this cycle:
DnD > IT > PS > Pes > UB
Diseases set up.
BB > SS > SS > Pes > Dump
Swapped out the 2nd BB in the standard cycle for Pes, however, by simply running through this cycle again, secondary targets receive only (assuming untalented) 3s of disease, so for about less than half a cycle there will be no diseases on secondary targets.
Going Pes > Dump > Pes might fix this problem and allow more SS into the cycle, but then it becomes difficult to squeeze DnD into the cycle. Reverting to the old disease set-up cycle fixes the rune problem, but there goes the whole purpose of us taking this rune in the first place.
These theories are all wholly based off the understanding that it spreads disease from your current target and refreshes diseases on the current target, in that order. If the refresh effect occurs first, then all targets will have their diseases refreshed from the spread. If the spread effect occurs first, then the other targets will only receive half of the spread.
So, which is which, Blizzard?

I think blizz just worded it wrong. I think it should read like this:
"Use: Your Pestilence ability now ALSO refreshes disease durations on your primary target back to their maximum duration."

Something I discovered as I was fiddling with the training dummies for a frost DPS spec...
Keep in mind that if your raid relies on you for
Improved Icy Talons
, that it only procs off of Icy Touch! Using this glyph to take IT and PS out of your rotations after initially applying the diseases basically makes those 6 talent points you spent useless.
Also, unholy may not get as much mileage out of this if they also have
Glyph of Scourge Strike
.
Blood, however, will almost always make great use of this glyph - only case I can think of would be a diseaseless rotation (if those even exist anymore).

The glyph fails if any CC is active as you would spread your diseases to the CCd target.
For blood it's to trade 1 HS + 1 IT + 1 PS vs. 1 DS + 1 Pest (and there's enough blood runes anyhow)
For frost it's to trade 1 IT + 1 PS + 1 BS vs. 1 OB + 1 Pest (and just because pest generates a death rune)
For unholy it's a bit more complicated since the glyph of SS is RNG for refreshing diseases and you would lose 1 BS in a perfect world of RNG with the glyph of diseases and in the worst world you would win a SS over IT PS BS. If glyph of SS fails less than every second rotation, then it's better dps than the glyph of disease
I really don't think the glyph is as good as it is on the first glance.

I think EVERYONE is ignoring how OP this is. At the current moment I am specced 13/51/7 with Glyphs for Frost Strike, Disease, and Obliterate. I am consistently topping DPS by hitting (for the most part) 3 buttons: Ob/FS/Pest churning out 3.5k dps on a target dummy. Of course, you start with IT/PS, but after that, you simply refresh your diseases with Pest and BAM, you're doing what other classes do in a pve encounter - hit 3 buttons. Mages, Warriors, Rogues, most classes are using only 3 or so buttons for the most part to do most of their dmg. DK's on the other hand, have to always keep up IT and PS, making us spend an extra few seconds and GCDs on something very very important to our dps. Instead, this compacts that so we can focus more on our hard hitting abilities. (Think of a mage, majority of their dmg is hitting Frostfire). My rotation then becomes Horn/IT/PS/FS/OB/FS/OB/FS/OB/FS/OB/FS/Pest and switch between Ob and Fs until you have 3-6 seconds remaining on your diseases. You may find other specs do a bit more dps, but also while learning new content, this rotation is very simple to use with alot of lee way in between to do other stuff. Also, I'm running in unholy pres tho blood does a bit more, however in these ulduar encounters, I need to be running back and forth alot.

This is an incredibly underrated glyph. Not just for Blood DK's, but all specs can benefit from refreshing diseases with one ability as opposed to reapplying each individual disease. So very helpful in just about any situation. The only way I don't see a benefit, is if you are destroying single targets very quickly.
