The Winter Hyacinth grows from ice on the Ironwall Dam and the Decrepit Flow in northwestern Crystalsong Forest. Pick four of them and go to Drak'Mar Lake, which is in northeastern Dragonblight on the border with both Grizzly Hills and Zul'Drak. Go to the middle of the ring of floating objects and use the hyacinths. The Maiden of Drak'Mar will appear and after a short dialog, the Blade of Drak'Mar will spawn next to her. Loot it to get the completion.

This quest, along with The Edge Of Winter, and A Blade Fit For A Champion, is one of three weapon-fetching dailies available to Argent Tournament Aspirants and Valiants. Each faction has its own version, though they're identical aside from the specific quest NPC. This particular quest is for Orgrimmar Valiants. The other versions are:

The actual quest is rather simple, you just read the quest and it'll tell you where you need to be.

Hyacinths grow only on the ice flowing from the Ironwall Dam, on Crystalsong Forest's northwestern border with Icecrown, on Crystalsong Forest's northwestern border with Icecrown. Gather the flowers and take them to Drak'Mar lake in northeastern Dragonblight, near its border with Zul'Drak and Grizzly Hills.

You just head to The Decrepit Flow in Crystalsong, and on the ice in the water and around the wall you can find flowers to gather, you need 4.

Then you just head to the Northeastern corner of Dragonblight, just above the Naxxramas (when looking at the map) and in the middle is a ring of flowers. use the flowers you gathered and some lady will pop out of the lake and you can then pickup the sword.

The flowers are on the ground near the Ironwall Dam in Icecrown.
Circle of flowers is on the lake at 93, 26 in Dragonblight.
Stand inside the floating flowers and "cast" the flowers and the Maiden of Drak'Mar will appear and offer you the sword.

The rep reward from this (and other valiant quests) is actually 63 rep with all horde factions other than the one you take the quest with, 250 for the horde faction the quest is from and 250 for the Sunreavers.

For example: If you complete this quest as a valiant of Thunderbluff, you'll receive:

250 rep with Thunderbluff
250 rep with Sunreavers
and
63 rep with Undercity, Silvermoon, Darkspear Trolls, and Orgrimmar

Note that while I did not receive any in-game notification of the rest of the reputation gains, checking my rep bars before and afterwards it is clear that you do actually receive them. Completing all 4 dailies will award you 1000 rep with each of Sunreavers and Thunderbluff and 250 rep with all other horde factions.

Don't walk out of the room as she's waffling on, though. Once you've used the hyacinths, they disappear and the sword's only there for a brief time. Fortunately, anyone can loot it while it's there, even if you don't have the flowers.

If you look at the plant in your inventory, it says that you need 4 to summon to woman :)

If the weapon is already there but about to despawn, don't use the flowers or you will lose both the weapon and your flowers. Just happened to me right now.

My tournament blade has gone missing and I need it for a match this afternoon.

One of the bards tells me that travelers used to present winter hyacinths to a lonely maiden in return for gifts. Those hyacinths grow only on the ice flowing from the Ironwall Dam, on Crystalsong Forest's northwestern border with Icecrown. Gather the flowers and take them to Drak'Mar lake in northeastern Dragonblight, near its border with Zul'Drak and Grizzly Hills.
