(and the spell has always been that way since it first appeared on thottbot)

So Krispee31 was right. The 40 simply was from an outdated, pre-PTR state tooltip. Don't trust anything that's written in plain text. For another example, remember the old tooltip of Mirror of Truth which claimed that it would only proc from melee attacks.

What do you mean Nathanvel about Mirror of Truth I see nothing wrong with the tooltip but that's just me...

So where does the quest start or how do you start it?

I just found out that when you received the 30 fragments and started the quest, you can also turn this in at the Archivum Console on normal mode (10 men Ulduar) and you can also accept the follow up quest Val'anyr, Hammer of Ancient Kings there. Nevertheless you have to complete this quest on heroic mode as it is said in the quest description.

I recently handed this quest in and there was a minor event were the Archivum Console spoke about the weapon (In the same way as the one in Hall of Stone but with no animated effects)
Here is a transcript:

Archivum System says: Fragment analysis underway.

Archivum System says: Object identified: Val'anyr, hammer of ancient kings.

Archivum System says: Created by the titans themselves, Val'anyr was given to the first earthen king, Urel Stoneheart. With the hammer he was to create and give life to the rest of his brethren.

Archivum System says: Val'anyr was shattered during the first war between the earthen and the iron dwarves. The weapon's remnants were believed lost in the conflict.

Archivum System says: Probability of successful repair by ordinary means available in this world is close to nil. Please hold while theoretical means analyzed.

Archivum System says: Powerful acidic content theoretically found inside the being known as Yogg-Saron would account for the liquification of saronite. Submersion in this substance might be sufficient to rebind an alloy of titan origin.

Also be aware that you cannot trade the individual Fragments which drop as loot. If they are assigned to the wrong player, they must be moved via GM ticket, just like any BoP item prior to 3.2.

The pieces fit together to form an elegant hammer. Its construction appears to be at least partially Titan in nature.

The console inside the Archivum might have some information regarding this artifact.
