To complete this achievment you need to kill Elder Nadox before the first Ahn'kahar Guardian spawns. Or he gets immune to all attacks.
This means you need to kill him in under ~15 Seconds, while he has 444k HP.

No, don't kite or tank the guardians. The trick is to pull the boss all the way to the entrance, then zerg him down. Just did it.

How to do this achievement:
Kill all the pulls in the big room before him. We pulled him to the doorway of the room and dps'd him to 70% before the first one spawned. The Guardians give an aura with a huge range. So you pull the boss as far away as possible (Corner of the big room) and have an offtank pick up the Guardians and tank them where the boss starts. (It's range is almost that long)
We used an Elemental Shaman and Feral Druid to handle the Guardians and then myself and the healer and 1 other DPS'd him down. The Guardians stack a non dispellable poison as well, so you still have to hurry a little bit.

We (me as prot warrior, 2 ret pallies, 1 MM hunter, 1 holy priest) did something else.
We cleared the left most path to him (without the pack to the left when you move down from the first stair). Hunter MD'ed boss to me and we ran towards instance entry portal as fast as we could.
When he caught up to us we started DPS and when we came to the first platform from the instance entry portal we gave it all and voilá - achievement done :)

the first guardian spawns at 80%, so just kite him to the entrance and kill him there before the first guardian is in range for the immunity aura. not really difficult, and not demanding anything special in terms of dps.

Did this today with a guild group even though it was supposedly nerfed, this strat still worked.
Pally tank, pulled from max range with Avenger's Shield and started running. By the time I got to the stairs on the other side of the room to go up and back towards the zone entrance, he started hitting me. I bubbled and kept running. When the bubble wore off I popped Seal of the Pantheon for extra armor, Holy shield, Sacred Shield and strafed so I could still run without my back to him. Healer was waiting at bottom of stairs and DPS started on him while I walked him backwards to the zone start, about 15 yards from the actual zone in.
We got him to 5% before he went immune from the Guardian that reach him. Our ret pally taunted him and pulled it back out of the room and the immunity fell off, and we finished it.
Our dps was probably avg 1700-1800, nothing extraordinary. He is undead so that helped me the Pally tank at least.
Perhaps easier if we prepared better for the guardian to reach us, but there you go.
Perhaps if you have a tank collect the Guardians out of line of sight of the boss he wont get the immunity? We didn't try it but perhaps that is the expected way to go.
Pally tank, Holy Pally healer, Ret Pally, Warlock, Spriest.

This boss was completed with ease, our group consisted of a Resto Shaman, Blood DK, Prot Warrior, Ret Pally and a Marksman Hunter.
We kited the boss back to the beginning of the instance with a simple trick, the tank stands as far back as possible, the hunter goes as close as possible, the hunter misdirect's, damaging him, as soon as this occurs, the tank needs to run back to the entrance of Ahn'Kahet immediately, the hunter should use aspect of the pack and accompany him at the same pace.
With all the trash cleared, we started with cooldown's being popped, and then we used Heroism, it was fairly close as the guardian was near us, but we managed to get it.
I was reaching 2.8k dps, Blood Dk, the ret pally 2.1k Hunter 1.9k, it's fairly easy, just need to plan things right.
Heroism is recommended for this achievement, as it helps alot.
This was a pug group, so it should be completeable by anyone.

Blizzard has stealthbuffed this boss, now the guardians are given sprint - increases movement speed with 200%. They rushed us to the beginning of the instance, we were taking it quite easy with the dps as normal, no need to rush we thought.
Anyways, we got very suprised when 2 guardians came rushing in from nowhere and destroyed the boss kill for us.

okay, this is something i tested out last week on an heroic run
we were going for the volunteer work achievement actually
but out of curiousity, i decide to check if i can cc the guardian on my moonkin
and you can cyclone the guardian to disable the shield effects which gives a rough 12 seconds for the group to dps the boss hard as they can until all the cyclones is use up on one guardian.
so my theory right now is this:
have 2 druids on cyclone duty and have the boss be dps down before the cyclones are used up.
not so sure if you can shackle the guardians and disable the shields though.

we went in with 1 dk tank, 1 warri ( off ) , 1 melee shaman und 1 holy priest and 1 warlock :D
the offwarrior and the priest took the guardians on the stairs to the left side of the boss
the dk and the rest went to the field on the right side of the boss` room.
more or less same way as tranami did

Went to do this today to help a mate get achievement (I had done it before the change, so was surprised to find the tank get 1 shot when we tried to kite).
We had a holy paladin healing, a ret, enhancement shaman, fury warrior, and DK tank.
Our strategy was to have the DK tank the boss at the bottom of the stairs in the room to the right of the boss, with the enhancement shaman healing, and the 2 DPS on the boss (he doesnt hit hard, easily healable by just about anyone). The holy paladin used righteous fury, and stood in the doorway to the bosses room. He would judge any guardian that spawned, and would tank it just by healing himself (Healing generated threat means that he isnt having to damage the mobs to tank them, so no danger of accidentally killing one).
It worked like a charm. The range of the guardians "immunity" aura is about 100 yards, so you just need to keep the distance between the tanked guardians, and the boss.

We just did this after 3.0.8 went live. We had our hunter use his tanking pet and stay in the room to trap / pet tank / sleep the guardians, while we tanked nadox down two flights of stairs in the direction of the next boss. The hunter was able to juggle the guardians long enough to give us ~45 seconds to a minute to nuke burn him down. Healing wasn't needed so much for this fight, so you healer can throw some nukes to make it move faster.
Kudos go to Niblick and Zinagaro on Thrall for figuring this out.

Yes, I can confirm that Blizzard has changed Elder Nadox so that if he gets a certain distance away from his room he will beserk, doing +500% damage and +150% speed. Our solution was similar to Gress, we tanked him on the circular platform that is to the right of the room as you enter into it and kept a Hunter inside the room who tanked all three Guardians with his pet. With enough DPS the Hunter will only need to live against all three Guardians for a few seconds, for which the new Deterrence is handy for. The achievement is more challenging now, but not too difficult. Don't try to kite him too far away anymore, as there's no distance far enough away to avoid the beserk that also won't allow the Guardians to get to him in time. A tank or someone to hold the Guardians is definintely necessary.

The kite no longer available after patch.
Here's a new tactic:
Prot. warrior has to pull boss to the the next room, 2nd tank have agro on guardians which has to stay in the basic room (over the corner) - that means - holy priest / resto druid hots both of them (has to run between them)
and 2 dps are supposed to zerg the boss down.
Not so difficult, just think :)
ad.) prot warr just use shield wall and last stand, healer is not needed there.

As of 3.0.8 this no longer works. The boss goes berserk and kills everybody (hits around 30k on plate, maybe more)
Quick and dirty guide:
Clear all trash before the boss
Have one (paladin or dk with proper stuff/spec) or two (heal/tank) players dealing with guardians in the boss room
Pull Elder Nadox in the opposite corner of the area before his room
DPS him when he has been placed correctly
Profit (or run)
Remember the immunity is about distance, not line of sight. Do not move Nadox from the corner or else you will get a few seconds of immunity that could make you fail.

Had the prot pally tank him back in the other room across the open space at the bottom of the stairs w/ a misdirect and had the DK toss on tank gear and pick up the guardians in the main room and pull em back to the corner where the boss starts out originally.
Healer stayed with the tank and the DK managed to survive the guardians long enough alone for the boss to be killed.

(Post kite nerf)
We did it with 2 healers, 2 tanks, and 1 DPS. The problem we ended up having was the little adds that spawn and buff each other being immune and getting on the OT, eventually killing him. After a while we stuck the priest on healing the OT, fading aggro off intermittently so the accumulated adds would go to the MT healer and we could kill them (they of course move very quickly).
The boss was on one far side of the room, the guardians on the other. The guardians spawn from predictable locations, with the first 2 or 3 (don't remember) on the same side. That's the side the OT was on.
In retrospect it probably would have been easier to stack DPS, but it's another thing to try. Doing it this way was both incredibly difficult and rewarding--probably much more difficult than intended.

Did this after the fix by taking the boss out of the room to the left and tanked him right by the stairs that would cause him to beserk and nuked him with any old plate wearer ot the adds that come keeping them out of range while u nuke

Repeated this achievement for someone a couple days ago. Pulled him out to the entrance as usual. Boss went berserk, tank popped cooldowns, DPS lusted and burned him down. Tank died when boss was at roughly 5%. Berserk is scary, but not insta-death. However this is potentially harder than doing it as intended. Just thought I would point out that it is still possible to use the old method.

We got this yesterday after a few annoying wipes.
Group consisted of:
Prot Paladin
(myself),
Unholy DK
(offtank),
Feral Druid, Ret Paladin
(healing),
Resto Druid
You shouldn't really
need
two healers, but it makes it easier if you're not burning the boss down to have the extra heals because someones going to be taking the small adds.
Clear the area outside, before the bosses room
Position offtank at the bottom of the long stairs (you go down them to reach the area you just cleared)
Other tank stays at the top of the stairs in the boss room (in this case, me)
Feral druid pulled the boss then popped sprint and ran to the offtank
Offtank taunts the boss, and DPS like normal
Guardians spawn, I taunted and pulled back into the bosses room near where he normally stands. This gives enough range from the boss so that DPS can do their thing.
Continue tanking guardians every 25%
Horay for achievement!
Whoever tanks the guardians should be fine if they can solo heal themselves. They hit like pansies (around 500) and seal/judge light was enough to keep my HP up.
The first few tries we kept him in his starting room, but that caused too many problems.

Killed him with 1 healer, 1 tank, 3 DPS. DPS DK tanks the guardians in the bosses room, right where the boss stands before the pull. The tank pulls the boss and runs him out the left door. We pulled him all the way to the corner of half oval shaped balcony thing right next to stairs. Boss never goes immune and we burned him down. Healer stayed with the main group, and the fight only lasted ~60 secs.

It seems alot of you are making this harder than it has to be..
Our group consisted of an arms warrior(OT), surv hunter, holy priest, ret pally and feral druid(MT).
Clear the room before the boss.
The healer sits at max range from MT in the middle of the room.
You
will
have a few seconds as the first guardian comes out of the boss being shielded but once the OT pulls into the corner the distance is fine.
If you don't burn the boss down before the second guardian comes out just have the OT come back out to grab the second and pull him into the corner.

After the patch this achievement became a lot harder.
We had a kinda "messed up setup" for this achievement. This is how we did it, probally you can use this tactic aswell, maybe with a little tweak, or get something out of it atleast!
After many wipes as the following
setup:
Healer -

We got the main idea of it:

We found it very very hard to do without a specified offtank class. So we tried to figure out a tactic for our group setup:

Nothing special, he just kept concentration up, and got threat on boss.

The Pull and Fight
So, basicly everyone except healer and tank positioned themself. Shaman stood on hes

the stairs when tank pulled. The tank then pulled, and ran with it all the way down the stairs, then to the

(facing from top of stairs at Elder Narox)
, to where the DPS's
stood.
He got to around
50-60%
, then the
first
guardians hatched. Shaman waited for the Guardian to go down the stairs,
boss was immune untill they were tanked by the shaman.
The shaman then grabbed the Gurdian
(only the guardian, not the small adds those ran to main tank healer)
, Main tank healer
got aggro from all the small adds
. They were killed as soon as
all
were at the boss. They went to Main tank, and
therefore
into the Paladins concentration,
and were instantly tanked.
Soon the next Guardian came. Same as before, boss was immune a few seconds, Shaman grabbed it. Adds came to Main tank healer, were tanked automaticly, then
killed
by the mage.
Then one more came, same as before.
I think there came
5
guardians
in total.
Hope this were usefull, even if your not using this tactic, this should overall clarify the entire fight.

It is possible to kite him away, but first you must clear everything in the instance up to Nadox. When the encounter begins get your tank to pick him up and kite him all the way to the entrance. Make sure that noone's doing any damage to him. When you get him to about the entrance simply tank n' spank him and it should take the mob too long to get there to create the shield. Boom, baby, strategy.

Ok, we did this a different way that seemed to work once we go the hang of it.
Had a Holy Priest, Resto Shaman, DK tank, Bookin and Elemental Shaman.
We started off clearing down the left side and into the bosses chamber.
The Elemental Shaman and Holy Priest ran back to the set of stairs that you had to go down to clear trash. The boss will enraged if taken up the stairs. We brought the boss just right too them, not up them. I being the DK tank, had to put on DPS gear as to take damage.
The Boomkin and Resto Shaman where in the main room picking up the guardians and the Holy Priest was getting aggro on the adds healing me and brought the adds into the DnD where they just died.
This took a few tries to get down right, but it worked very good.
Tips:
-CC the first one via Roots, or Shackle if you have a Holy Priest in the back.
-This might sound odd, but make sure the tank on the boss is actually taking damage so that the healer on the tank can get aggro from the little adds. If the healer on the tank can not get aggro, the adds will just own the people on the Guardians while they are immune.
-Disease and Poison totems based on the area of the Shaman. Disease on the boss tank, poison on the add tank.
Using this plan, we are able to DPS the boss almost the full time. The only time he becomes immune is when a Guardian spawns in the back.
You could easily replace the Shaman or Priest with another healer and pretty much do it well, but you'll need two tanks. One decently geared DPS is all that is needed on the boss.

We did it with a fury warrior in prot gear, feral druid, holy paladin, resto druid and a mage. I play the warrior, and it was easy. We used me to tank the Guardians, and we used Sirupsen's tactic. GL to all you achievers out there!

We had the ret pally position himself outside the area before the boss, and have the hunter md the boss to him. Once he's got comfortable aggro he pulls way back to the corner under the stairs. I position myself in the boss's room and grab all the guardians when they spawn, hugging close to the exit. Healer focuses on the pally.
Main reason is they can zerg the boss down, while I take little to no damage from the guardians.

Just completed this (and most of the other Achievements in AK:OK).
Group: Prot paladin, Unholy DK, Mage, Rogue, Holy Paladin
First try we tried keeping it in the first room. Bad idea: Whilst the first room is long enough (just) the few periods of immunity the boss got made it too hard to deal with.
Our eventual solution: the DK tanked the boss at the bottom of the long flight of stairs, in the previous area. I stayed on my own in the boss's room, tanking the adds, with seal of light/judgement of light I managed to stay at a reasonable level of HP. The DK survived tanking, DPS burned the boss, achievement earned.

Just did this with a Resto Druid, Feral Druid, 2 Balance Druids, and an Enhancement Shaman. (Our Shaman tanked Nadox) and we downed it in a few tries pretty easily. Definitely an easy achievement once you get that strat down!

Some guildies and I were runnning this last night, and we didn't have a second tank. What we did have were 2 warlocks, a resto shammie(me), a feral druid, and a hunter, all in 25 man gear. We had the warlock summon a voidwalker, and then had the hunter misdirest to the voidwalker, who was standing in the area at the base of the stairs, along with me and the other dps. The elder runs there, the feral druid stays up top to grab the guardians, and I pop BL, heal the group, and voila, we drop the boss in under a minute. All done with no official second tank! LOL, I love this game.

Done post 3.0.8: 1 hunter 1 shadow priest 2 shammy(1 healer) 1 warr tank. Not pre-sorted group, just randoms. MD pull on warr that is at bottom of stairs. SPriest shackles 1 guardian, hunter traps 2nd, kites 3rd by the time boss is dead. Good luck ;)

Completed this achievement last night with the following set up:
Prot Paladin
Prot Warrior
Arcane Mage
Afflic Warlock
Disc Priest
I saw this mentioned above, but I thought I would reiterate it here: If you tank the boss where he stands, you can pull the guardians to the wall directly opposite and that is far enough away that the immunity effect can't reach. This way ensures that you don't have to deal with the iffy distance of the enrage. Paladin tanked the boss, and the warrior (myself) tanked the adds. I focused the boss and watched his health and started running towards him when he was about to spawn the adds so I could pick them up. Immunity never lasted longer than a couple of seconds. The priest mostly healed the paladin; the guardians don't hit that hard, but he would toss a PW:S and a few spot heals on me as I ran close to grab the adds. The little bug adds pretty much healer aggroed every time, but he dragged them into the paladin's consecrate with little trouble.

We did this achievement with the following composition:
Unholy Tank Death Knight (ME)

The only thing that matter here, is that you don't LET the boss get the immunity. I tanked the Guardians, The other DK tanked the boss, and dps'd. It simplifies the fight GREATLY. Caylie's Method is basically what we used with a small exception. We had the boss faced towards that enclave there and the group stood behind the tank. This helped GREATLY with killing the swarmers that spawn. Also, instead of moving the guardians out of the room, I just tanked the guardians in the room with no assistance whatsoever. (Although some
items
and
abilities
can be
used
to
slow
down your death).
To clarify where he should be tanked: AS you enter the instance, head through the first doorway, clear that room then head left. You need to head down those
stairs (the ones your going to be tanking the boss next to)
and clear the room. Proceed to follow the right wall to another set of stairs which is the entrance/exit to the bosses room.

Did this a few weeks ago with a somewhat... Uncanny... Group comp.
We had:
-Demo Warlock in PvP Gear (Off Tank)
-Feral Druid (MT)
-Resto Druid (OT healer)
-Holy Pally (MT Healer)
-Hunter (DPS)
We had the Feral Druid tank the boss right where he starts, and had the hunter just DPS the boss the entire fight. I (Demo Warlock) would grab the adds and take them to the stairs in the previous room. The Voidwalker would proceed to tank them there. When a new add would spawn, I would run back and grab it, then sick the voidwalker on it. In the end, Voidy was tanking 2 of them, I tanked the last one and we downed the boss. The Holy Pally would just stand with the hunter in the first room, and the resto druid would stand on the stairs near the voidwalker the entire fight.
Fairly simple achievement, both our MT and our Druid healer were geared in Heroic Blues and Purples... It just takes a tricky raid comp. DPS isn't really an issue on this fight (its ALL about positioning) so feel free to take a 2-tank 2-healer group if need be.

We did the achievement doing the following:
The group consisted of:
-2 Tanks (1 Off-Spec)
* Death Knight (Main-Spec)
* Feral Druid (Off-Spec)
-2 Healers (1 Off-Spec)

* Resto Shaman (Off-Spec)
-1 DPS
* Destro Warlock
This is a map indicating where people should be positioned and where the boss should be.
Respect Your Elders Positioning Map
* We cleared all of the trash in the room to the right of where the boss stands. We then had an Off-Spec tank (Feral Druid for us) go into Cat form, attack the boss and use dash. (Could also be pulled using another method).
We tanked the boss in the back corner to the right of the stairs you head down going towards the boss (Indicated on the map). In this position, you want 1 tank, 1 Main-Spec healer and your DPS. Your healer will need to not only heal himself but also the tank as some adds will probably come through the door and start attacking you as they become immune as soon as the Watcher spawns. Healing will get very intense during the fight due to a lot of adds becoming immune and the second tank can not produce aggro on them. I used Beacon of Light on the tank and spam healed myself to keep us both up, a Priest using Binding Heal will also do the job efficiently.
* The second tank and healer then stood where the boss is currently positioned before you pull (Indicated on the map). The tank picked up what adds he could as they we're spawned throughout the fight. Once the first Watcher is spawned, the boss would usually become immune, this does not happen due to the range between the boss and the add. The tank picking up the add needs to be sure he does not kill the add, otherwise you will not get the achievement! Usually their is only 1 Watcher that will spawn, but if you have a second, the boss could become immune as the add may be too close, if the tank is not positioned right. With a slight correction of positioning the boss will no longer be immune and the DPS can finish the boss off.
This was the best tactic for us after trying a few different ones.

Alright, this tactic my group used worked out just swell, didnt have too much trouble doing this, anyways, you want your main tank(me, the prot warrior), a dps tank (ours was a warrior), and two dps and one healer
Have your dps tank use heroic throw on the boss (if hes got around 24k unbuffed you should be fine, with a good healer that is), and have him drag the boss to the opposite side of the room, the one you entered in, and keep him there.
Have the healer stay with your dps tank, behind him, next to him, whatever it may be, and have the dps on the boss. Kill the little spider dudes with aoe, they only got 250 hp so its all good. The main tank, the one who is going to get the guardians, stand where the boss starts at (even further, try to stand in the window behind him), the first two guardians should spawn near that, keep em tanked as far away as possible ( when i tanked them, elder nadox wouldnt even show up on my screen, he was
that
far away, and i didnt have to leave the room). The 3rd guardian is going to spawn a bit closer to the rest of the group, which should be at the opposite end. Have the main tank run to the 3rd guard and heroic throw/taunt him off and just run straight back. As a main tank i had 29k unbuffed, and the guardians tickled me, didnt need heals at all, and if anything you block and dodge the moves, you really dont need healing, cause the healer should be focused on your dps tank. Blow down the boss and you should be good, if the guardian tank needs heals for any reason, an offspec healer can go throw up dots, or the main healer can go and offspec heals can cover for him for a few seconds, though its not likely. Throwing up shield wall also helps a lot for the guardian tank.
Nothing really else to it, if the dps tank needs to put up a shield then do so, hopefully it wont sacrifice a lot of dps. Gl with the achievement guys :) hope this helped!

I just stayed in the boss room and OT'd in various pieces of tank gear I had as Fury 2h and Shield.
The Guardians were barely hurting me, popped Shield Wall and Shield Block just in case when second one came.
The Druid tank took him out of the room and stopped kiting him once they got to the bottom of the stairs in the other room that lead up to the entrance.
Easy as 1-2-3.

How To Easy Way:
First of all this strategy was done doing a 4man with just 1 dps so it of course will work even easier with another extra DPS.
How this work first of all you need to keep the boss and the guardians as far away as possible obviously since if you do not the guardians give immunity aura.
Team: 2Tanks 2Dps 1 Healer.
Healer can be anything we used a priest.
DPS anything.
Tanks: Here is the trick what you need is a tank with a good mitigation/avoidance with shield included i would go for a warrior (me in this case) since he can tank the adds forever. And another tank i would go for a pally thats what we used since of the aoe for the little spiders.
Now the team is set up
How to:
Before doing anything CLEAR everything in the right zone of the boss.

First of all the Pally will be tanking the boss he will pull it and run towards the "red" dot in the map, hit the wall and when the boss stop he moves away from the wall so he will be in the middle of the path of the little spiders (not moving the boss just turning him arround) and staying in the "yellow" area.
The priest and dps when he turns arround the boss will go to the wall and put the back into the wall so the spiders will always find the tankadin first and will be even easier to target anything in the healz to remove it.
Spiders----Tankadin----Boss----HealDps---Corner
Now the guardians. Pink Circle is the warrior tank (thats me so i know it works) you stay arround that area and TC the little spiders so for the beggining they wont annoy the group
-When the first guardian spawn you tant it do a few threat moves and hold it use SBlock. and Demo Shout.
-Second guardian then again you taunt you use the block if you have it. Demo shout again.
-Third guardian taunt pop shield wall to avoid damge. Block if you have it. and Demo again.
Use enraged regen when you hit half HP always Shield Block and wait until you get enough mobs over you to make the Shield Wall be worth it.
My gear is a mix of nax,voa 10/25 gear so im not really that overgeared to do this.
The guardians hitted me for arround 500~~ dmg but with enough mitigation and spamming the block you can tank them for 2-3mins without any problems

We used the map posted in the comments above. it was clutch. We had the shaman put on his pvp gear and a shield, he set up in teh doorway to tank the Guardians. Everyone else stacked on the tank in the far corner. I threw tricks on the tank and then pulled the boss. DPS'd almost the whole way back to the tank. Shammy popped Bloodlust, we all blew DPS cooldowns. He had no trouble tanking one guardian. When the second guardian popped, he blew his Earth Ele totem, and that was enough for us to finish off the boss. One minute and 8 seconds. In retrospect, the ele totem probably could have tanked the adds all my its lonesome.

As of 3.2, this is still doable by kiting the adds down either side.
We did this with a feral druid, holy pally, ret pally, lock, and warrior tank. feral druid taunted the add and ran it back. ret pally and lock dpsed. second add spawned. ret pally ran it back and healed the feral tank in the back as the lock nuked.
i, being the healer, had to tank the small adds this way, though. nearly died a few times, but eventually got him dead. very effective strategy. just need a competent group.

Just did this achievement for some guildies. Group comp:
Prot Paladin (well geared)
Holy Paladin (Fresh 80)
2x Destro Lock (Both well geared)
Boomkin (Well Geared)
The holy paladin stayed in the room that the boss starts in and we pulled the boss outside the room to the other side of the stairs and blew the boss up. The Holy paladin had to use Divine Protection, I was tanking and I didn't have to use any cool downs, just had to dispell the disease the boss puts on asap and I judged light that kept my health above 60% the entire fight.

Hi there, Ive just done this achievement in the newest patch...
I am a Holy Paladin.
The tank pulled the boss back into the other room, just to the steps where you come down to the boss, I, went to where the boss orgrinal stands, and with using Righteous Fury, Tanted the guardians and used Judgements whilst healing myself / Sacred sheild.
Only 3 guardians spawned in the time we got the achievement, it was quick and easy.

had our boomkin drop trees on boss and run like hell to group all cat form and sprint like, we met him at the end of his sprint duration(we wiped 3 times getting it right).
the above info about his enrage is accurate, and quite brutal, hits ulduar25 geared tank for upwards of 15k after various damage reduction/absorbes.
as to strategy, we just burned him in about 26 seconds, rotated Divine protection (paladin) and pain supression (me), and shadowpriest popped divine hymn at one grim point.
moral of the story is that if your group can push 7ish K dps, and you have smart cd communication btwn tank/heals, this is doable the old way...for those who miss how patchwerk used to hit our tanks :)

We attempted this today, using the method to keep the guardians in the room they spawn in, and tank the boss in the next room. We didn't kill any guardians but we didn't get the achievement, very odd.

Just did this, RIDICULOUSLY easy.
Comp was:
1 resto druid (myself)
1 feral druid (tank)
1 disc priest
1 hunter
1 ele shaman
All we did was sit myself in the main doorway to pick up the adds when they spawned, had the hunter MD to the tank who dragged the boss to the bottom of the stairs, and then they burned him down using heroism. I just stayed in tree form and spammed myself with heals. Never had a close call in terms of me dying. As long as the DPS is solid, they can kill the boss in under a minute and there is no chance of the healer tank dying.
This would also be very doable with a holy pally, resto shaman, maybe even a priest, though having some armor to mitigate the damage from the 3+ guardians and the little spawns is very helpful.

The off tank does not need to be healed with decent gear and spec'd.
My group had a fury warrior switch to prot and put on some prot gear. He stayed in the room and didnt receive any healing the entire time. The rest of the group (MT healer and 2 dps) took the boss to the "secret" room/balcony area spoken of above.
The key is to keep the boss FAR FAR FAR Away from the guardians.

We had a Druid on guardians, Just DPS'ed the boss while Druid is handeling the Guardians, And Ding achie done :)

Another better (imo) method.
Group Comp:
Prot Pally(ot)

What happens is the pally off tanks b/c he/she can heal themselves and since the add isn't taking any damage just the heals will hold aggro.
The other thing we did was to tank the boss on the opposite side of the room instead of the adds. This puts the OT exactly where 75%(we had 4 total for the fight) of the adds will spawn. The placement of the OT where the boss stands means he doesn't have to run very far to keep the boss from being immune(the cause of all my previous wipes.
This is probably the best construction i would recommend

we done this achiv by the setup= 1 tank 4 dps;
when u begin the instance move to the left side way of the instance the one usually u wont take. clear each adds there
before u engage the boss u may notice there is big spot on the edge before entering boss room. i as warrior tank
kited the boss there and with 3 dps burning him down. while leaving 1 dk (dps he used frost presence)
to pick up any guardians that spawns. with no heals i barely holded up as when my 3 dps burned him i had about
only 3-5k or so. and thats how we did it.

was in a group with a Huntard and tried this the other day. well, for some reason, the group listened to HIM, and we tried to zerg the boss in 15 seconds. then when the first guardian spawned, no one attacked it. and we wiped. bad idea.

Did this today easily. Brought 2 tanks, 2 healers, and one dps.
Have one tank bring the boss out of his room and into the corner by the stairs. The dps and a healer comes with him. Have the other tank and healer stay in the bosses room, and let that tank pick up any guardians that spawn and tank them where the boss is originally positioned.
Doing this the boss only went immune momentarily once or twice, and we got the achievement. You will want to make sure you bring a good dpser.

We did it easily enough with two tanks. The warrior tank took the boss for a walk to the stair bottom with the others, and the pally tank stayed alone in the main room, pulling the guardians to the corner. That the pally tank was overgeared made it very easy, she would pop cooldowns serially and take no damage whatsoever.
One small piece of advice would be to have the other 3 stack on the boss tank, so that he can pick up the annoying mini-adds that will escape the guardian tank, they just pop up over too much real estate for one tank to cover 100%
/as an aside, don't do this with the LFG tool, the chances of finding randoms pugs who want to wipe-wipe-win, is very rare :)

Just did this no problem without a proper offtank. Went in with 3 guild members and a pugged dps.
had the following set up
-Arcane mage (me)
-Prot warr
-Holy pally
-Affliction lock
-Frost dk (dps and the pugged member)
the dk was kind of dead weight, and didnt want to tank the guardians so our warlock went in with voidy and tried to hold the guardians as they came out. tank went in and pulled to the side room as illustrated in other posts. voidwalker held fine while we burst him down just as the 2nd guardian was coming up. dk really didn't contribute much dps wise but with an icc geared tank and mage we took him down pretty quick all the same. so don't think you need an offtank to do this we just made it into a burn fest, would've been easier with a plate wearer picking em up but we did fine anyways.

We just did this very easily. I'm a ret pally with my second spec as holy. We had a Tank DK, a Mage, a holy pally, another ret pally and myself. I switched to holy, and ran to the very end of the room where he sits. The tank took the boss to the opposite side. Since the guardians usually spawn closer to where he sits, I just sacred shielded myself and taunted them all to me as they spawned, healing through it. They dont hit hard, and if you have heroic badge gear you should be fine.

There's no reason to bring anything more than one tank and one healer for this achievement, as long as you have good dps.
I did it with the following comp:
DK Tank (Blood) - me
Disc Priest
Boomkin
Rogue
Arcane Mage
I aggroed the boss and pulled him to the bottom of the large staircase that leads into the room with trash before the boss (on the right). The DPS were waiting there, and opened up on him as soon as I got there. The healer stood where the boss was, got aggro on the guardians as they spawned by dotting them, and then just healed himself. I think we only got one guardian, and they hit so softly that any decently-geared healer can heal themselves. The boss was dead in maybe 20 seconds...easy.

Before i go into details, i dont care if you downrate this comment or not, i just want to give ppl good advice on how to complete this Achievement.
Simple Setup, and Simple task.
Bring
2 tanks
1 Healer
2 DPS
Have one tank the boss. He should tank him where he stands but...more back, just hug the walls and have the boss away from raid.
the others should do what they are supposed to do, healing and DPS.
The Offtank should just pick the Add/Guardian when he spawns, Easy one, just pull the add, to the opposite of the room, far to the wall but dont kill him, because as a tank, you can dps him down.
But, just offtank him, and w8 for the boss to be down....and You have your achiement.
The healer, should stand in the middle, if the offtank need some healing, he can reach him, and back to the main tank. Believe me, its very easy.

I came up with a strategy of my own, because we didn't have an off-tank. It worked really well for our group. We had a (not-so-great) DK Tank, and a Druid Healer. Mage, Warlock, and Rogue (Me) for DPS.
Please look at this image for a better explanation.
You need to clear most of the trash between entrance and the boss room.
Everyone should DPS the boss as much as possible.
Use all your CDs (except for Tricks of the Trade, Sprint, and Evasion).
The Party needs to avoid any AoE's, and make sure no one pulls the Guardian when he spawns.
The Rogue needs to grab aggro, use Evasion and Sprint, and pull the boss out of the room back they way you came in.
The healer should put some HoT's on the Rogue on his way out of the room.
At the top of the large set of stairs, blind the Guardian.
The Boss should die soon after, but you should get farther away from the Guardian, just to be safe.
At the worst, (the Rogue) might die (I didn't), but the boss will probably be dead by the time the Guardian gets back.

This achievement doesn't even need two tanks, nor someone with heavy enough armor to off tank. I did it today on my mage. I just speced into my PvP frost spec and kept the guardian novaed practically 24/7 (frost nova, freeze, shattered barrier) while everyone else killed the boss.

I tried this last night as a Paladin off-tank, and I grabbed the Guardian. Everything was going well until the boss was at 10% or so. Unfortunately, the little spider adds killed our healer, so as a last-ditch attempt to buy some time, I used my Divine Shield. The Guardian despawned when I did so, and we did not get credit for the achievement. We're going to try again tonight without Divine Shield, but I will tank the boss and our warrior tank will grab the Guardian.
edit: Attempt number two worked perfectly. I switched my weapon to the
Argent Lance
so as to not do more damage to the Guardian than necessary, and didn't use Divine Shield.

Just did this in a random today with my newly dinged hunter. The rest of the group fought the boss and I kited the guarding back to the beginning of the instance. There's only 1 guardian spawn now, so it's fairly easy.
The guardian does have some kind of a dash ability that might make it difficult to kite. if you have a survival cooldown, you might need to use it for then. It can be frozen.

As of 3.3 this achievement is now doable in a random. Simply pull him to the south eastern corner of the room right to where the boss is, and have another player keep the guardian inside the boss room. Done & Done.

My group had a Shaman, Hunter, Disc Priest, Warrior and Mage. We pulled boss up the webbed platform, and had hunter kite the Guardian, got the easy kill and achievement. LoS the boss didn't work so we went distance
