Tier 16 is a 5-piece set obtained with tokens from 圍攻奧格瑪.
There are three different colors: 528 gear from LFR, 540 from Flex shares colors with 553 from Normal, and 566 from Heroics. This is the LFR color.
These tokens are turned into tier from vendors at your faction's shrine:
Alliance: 薇碧茲‧歡哨 (LFR)
Horde: 布雷里茲‧輝刃 (LFR)

Tier drops are as follows:

Helm: LFR tokens from 『嗜血巨龍』梭克 (Wing 3) in 圍攻奧格瑪.
Shoulders: LFR tokens from 攻城機匠黑引信 (Wing 4) in 圍攻奧格瑪.
Chest: LFR tokens from 傲慢之煞 (Wing 1) in 圍攻奧格瑪.
Gloves: LFR tokens from 納茲格寧姆將軍 (Wing 2) in 圍攻奧格瑪.
Legs: LFR tokens from Klaxxi Paragons (Wing 4) in 圍攻奧格瑪.

Matching accessories:

Waist: 葛拉卡斯 or 卡爾洛斯‧地獄吼
Feet: 伊莫爾西斯, 葛拉卡斯, or 馬可羅克.
