For Secrets of Azeroth to craft Reforged Tyr's Titan Key you need to first turn in 復原的黑曜鑰匙 to Forgemaster Bazenthus, that will unlock quest 重鑄之鑰 at Weaponsmith Koref next to him.
Accept the quest, talk to NPC to get started, use extra action button near forge, loor the key from the middle of a forge, done.
