Bly is the leader of a group of captives in Zul'Farrak. Once you release the group, a load non-elite mobs and a few elites spawn at the bottom of the temple and accend the stairs in several waves. STAY ONTOP OF THE TEMPLE! Once a certain number of waves have accended, the npc group will head down the steps while a few more waves come. At the very bottom are 2 elite mobs.

Once you've killed the 2 elites, the npc group will stand around. At this point, talk to the goblin FIRST and he will go blow up the door leading to the last 2 boss's in the centre of the instance, then he will run off. Then, have someone talk to Bly, and after a short convo, the npc's will turn hostile. Kill them and your done

Here is a nice trick I found:
First, talk to the goblin so he will open the doors.
Now, wait for the characters to start using their Hearthstones, now talk to Bly. What will happen is that the other NPC's will HS away and only Bly will remain and fight.

Fun fact, you can have Bly help you kill his allies by simply talking to him twice, he will stay and help you fight before turning hostile.

Note: If you kill all the mobs down the steps right away, the NPCs won't move and it'll look like if you bugged them. But if you wait a few minutes, the event will restart.

A good idea is to talk to the gnome before he takes his Hs, We didnt, and we couldnt get the door up. And good luck ^^

I just did this event... went down and AOE'd all the trolls dead at the bottom. The event never recovered. I went AFK for 15 minutes and Bly and his group were still at the top. It seems like if you don't let them count the trolls or something they won't ever head down the pyramid and wave 2 won't start. Go slowly.

you can glitch this guy's party to attack him, you gotta get them to attack a nearby elite, then talk to the sergant and then the team will attack him. I'm not so sure if they will go hostile after helping you kill him, but it didnt for me so it might not.

If a priest mind control him, (yes you can MC him lol ) and when the MC is down he will reset, funny..

I just had a group wipe on the stairs and then came back, killed everything and the boss that spawns afterwards and Bly & Co. were nowhere to be found and we could not finish the instance. Been playing since Vanilla and have NEVER seen this happen. Has this always worked like this? Wipe on the stairs and you can't finish the instance? O_o
