Here it is two years later, and I can confirm that not only does this problem still occur, it is indeed caused at least in part by one or more user interface addons. I haven't identified a specific addon(s) that causes it because, frankly, it will be a PITA to enable one addon at a time and then replay the quest to observe what addon eventually makes it fail. I asked Blizzard to help me understand what an addon could do that would cause this one specific quest to fail, out of hundreds that didn't fail; I had hoped such information would implicate an obvious suspect, but of course no useful answer was forthcoming. If I do make that effort and catch the suspect, I will share it here.

As of today I can say that it isn't an add-on only issue. I have no add-ons, never have. Still didn't drop after killing him.
