Judging by the non-heroic version, 2min cooldown.

While not that interesting for pve, for pvp this heal is godly. It bypasses spell locks and silences, it is instant and on a moderate cooldown. The passive mana regen is a "Blessing of Wisdom in a trinket", and eventually useful. It is doubtful this will surpass the usefulness of the new season's Battlemaster, but for Humans this is shiny (Not needing a medallion. this is a great trinket candidate for the 2nd slot with BMaster).

Just to state the obvious, unless you have need of the proc (e.g. for PvP or for some future Azgalor style boss), this trinket is basically inferior to the normal (245) Solace of the Defeated/Fallen. Solace has 150 static SP and 128 mp5 when ramped up--which is is for almost all ICC heroic fights. And if regen is your focus, this trinket is still at best on par with (and for most classes, strictly inferior to) the Spark of Hope from 10m Ulduar.

Folks in the comments are comparing this to Vial of the Sunwell, which is apt. Another comparison would be to Binding Light/Stone, another poorly itemized mp5 trinket which was effectively better than nothing.

Even though this is an itemlevel 264 (non-heroic) / 277 (heroic) healer trinket, it seems to me that most healers will be better off with other, easy to get trinkets. I'm not sure if this goes for all classes, but as a resto druid, I'd be far more happy with the itemlevel 232 Ephemeral Snowflake from HoR HC, and even with the Spark of Hope from Ulduar.

Granted, the instant heal on use can be very nice (if Swiftmend is on cooldown or the target has no HoT applied), but the cooldown is too big to make it really useful for the average PvE bossfight. Compare the 76 (non-heroic) or 86 (heroic) mp5 of this trinket with Ephemeral Snowflake, which can reach up to 165mp5 theoretically, and usually goes well over 100mp5 in a practical situation, or spark of hope which will give even more mp5 than that (but doesn't have an on-use effect), you sacrifice a whole lot of mana regen just for an instant heal you can use 3 times in an average bossfight.

Of course, the effect of the aforementioned trinkets may be lower for other healing classes. For example, my limited knowledge of holy paladins makes me think the Ephemeral Snowflake can't possibly proc 3 times/second for them, as they don't have HoTs. The effect of spirit from the Spark of Hope will not benefit paladins or shamans either. But still, I think for an itemlevel 264/277 trinket, the effects are a bit disappointing.

Strange and special trinket. I subscribe the said avobe, and yes, this is an interesting PvE trinket for holy paladins, and perhaps only for them. It is a great PvP trinket (see other comments above).

Let me show you why this trinket is more interesting than 1 empty bag slot:

First of all, ignore spell damage or intellect once you have a lot of both stats. Holy paladins are spammers. If they dont, tanks die. Despite pleas or illumination, all their mana regeneration relies on mp5. But, as said above by others, this trinket can be used at full power while pleas! And this is a very interesting use for it. Holy paladins want intellect, but 40k mana raid buffed is enough mana to spam heals without stop. The question is then, how much mana do you earn wearing this trinket?.

Holy paladins normally overheal. Because of that, mp5 instead of spell damage is not bad during very long and hard fights. Please bear in mind that once you have a minimun spell power to be overhealing a lot, spell power is not efficient. For ICC25 heroic, that is around 3000 spell damage. So spell damage reduction is not important, when your raid is a good raid and your spell power is already high.

The heal given by this trinket is an intermediate flash/holy shock/holy light, a little higher than normal flashes, much lower than holy lights. But it is similar to holy shock. It has a higher range than holy shock (40y). So we can say that its typical price should be 1000 mana. It doesnt scale with spell power, so this is a VERY bad point, because when you have a powerfull demon warlock in your raid, holy shocks hit greater than this heal.

During a typical 5 minutes fight you may use it 2 times with high success in the sense to save people otherwise DEAD. This is great no more words needed. It scales as logical with longer hard fights and with phase 3 mana burning bosses (Arthas, Sindragosa...). But, anyway, lets talk about rough mana numbers. After 5 minutes this is 86mp5 = 5160 mana and 2x1000 (suggested price for the 40y instant heal usable silenced / debuffed / while moving) this is 7160 mana. Notice 1000 mana is probably a very under estimated price.

Solace of the Fallen gives you 7680 mana. Very similar numbers. But you need to use holy shock in your rotation always...The key difference is that with solace of the fallen you gain 150 spell damage, that is, 5% of your spell power or less. Everybody knows that holy paladins overheal four much more than this. The effect of 150 spell damage on Sacred Shield is more intersting, but not very impressive. Using Bauble of True Blood you gain the abillity of healing hard while moving, silenced, debuffed or OOM and as far as 40y, where only your beacon of light reaches... This trinket is very usefull for a holy paladin, when moving and during very hard battles with very hard phase 3. It is, as somebody said above, a nice trinket for Sindragosa 25 heroic. But it is valuable with Arthas too.

If your idea of healing is winning at the reccount healing meters, this trinket is nor for you. If your idea of healing is related with sinergy (let other healers heal for you where they are better than you) or being recursefull, intelligent, saving lifes otherwise lost and you love sustained mana efficiency (independent of healing rotations, tactic situations or technnic problems: being silenced / debuffed...) this trinket is four you.

This trinket is not good for Valithiria Dreamwalker. XD.
This trinket is not good for flash of light + holy shock, spell power builds.
The trinket shines when you reach haste cap (675 haste) wearing every possible item for mana efficiency, adjusted spell power and you already have the maximun logical intellect (around 40k mana raid buffed). Then you normally heal with holy light and occasional holy shocks while cast flash of light as merely an entertainment. This trinket gives you a new way to heal on panics, and sustained mp5. Very interesting.

And finally, this trinket is cool because it drops from Lana'Thel.

This trinket is very useful for Shadow Priests in arena. The ability to send off an 8k heal off the GCD or while under the effects of dispersion is incredibly useful. While it is a PvE drop, it seems far stronger in PvP

Currently on the ptr the mp5 is replaced with +172 Spirit (Heroic version)
IMO it makes this trinket a lot stronger for all healer classes with the new Spirit/Mana regen changes in 4.0 and Cata.
Also the Spirit on this can be reforged into any other stat i.e. Mastery, very nice.

*Edit* Also the Heal is changed to a flat 8000
