Reignac and Metabii's comments intrigued me about this item so I made a worgen just to test it out. I thought it was a little strange that it affected Sylvannas and thought maybe it works on other npcs too, so I did some testing with the rapier and tried using it on various npcs in Stormwind and surrounding zones. This is what I can confirm so far as of 5.3:

Yes it does heal Sylvanas for about 30%(9000) of her maximum health per tic so be careful where you aim!
It does NOT work on yourself or other players.
While castable anywhere, it does NOT work on npcs outside of the Gilneas questing area such as Stormwind Guards, King Varian, etc....
