According to the 3D viewer this has got to be the largest "dagger" I've ever seen.

Pop a + Mighty Spellpower enchant on this bad boy and you're looking at a bonus of +418 spellpower or pop on a Black Magic enchant and get a extra bang for your buck proc.

Overall this is a freakishly fantastic rep based reward that I'm going to be grinding for.

As far as I can tell, this is the best caster one-hand available via quests, rep, etc. This and Ward of the Violet Citadel probably the way to go while waiting for drops or PvP gear.

Easily one of the best rep rewards, especially for revered. Chances are you won't swap this out for some time. Took me a while before I remembered it was a dagger after getting it though, damn thing is bigger than some swords.

This Weapon is a MUST for any dps caster and should have no excuse in not getting it considering it it the best dps caster dagger pre Naxxramas.

Combo it with http://www.wowhead.com/?item=40698are or with http://www.wowhead.com/?item=37134 and zou will be fine.

This weapon is almost identical to http://www.wowhead.com/?item=37377. In fact, they even have the exact same model. When I compared the two, I came up with this:

The Netherbreath Spellbalde gives you 26 more spirit, and 9 more crit, meaning that it could be a better choice for healers and maybe warlocks.
The Flameheart Spell Scalpel gives you 1 more intellect, 3 more stamina, and 34 hit rating.

Because of the extra spirit and small amount of crit, some healers and warlocks may choose the spellblade over the spell scalpel. Personally, I would take the spell scalpel over the spellblade any day of the week. The +hit rating on it is invaluable for taking down raid bosses.

When considering a weapon for my first Naxxramas run, I originally settled on the Cursed Lich Blare from Novos the Summoner in Heroic Gun'Drak. After a short look at rep rewards I quickly decided to get this instead. I am running lots of heroics for gear anyway and doing Heroic Violet Hold for the Mark of the War Prisoner will get me tons of rep since it never seems to drop. At about 1.3k rep per run you can get this revered easily. Also, as previously stated, the head enchant opens up at revered too, so you can pick that up while getting the dagger. On top of the amazing stats, you can throw an enchant for 400+ spell power. Plus this has an awesome skin!

Took me roughly 9/10 Heroic runs and part of a naxx raid while wearing the tabard to get revered as a mage.

It's a must for casters to get revered with Kirin Tor ASAP, or even exhalted. You get head enchant, a mainhand and a robe.

Considering the other options and ease of getting this dagger, this is probably the best bet for a DPS caster pre-Naxx. The Cursed Lich Blade and Netherbreath Spellblade are comparable, though they come in around 20dps less on spreadsheets. So if you're a Mage, Lock, Boomkin or someone wanting a MH with some spell hit, this will get you in the front door.

easily lasts until naxx gear. hit is always a welcome attribute to dps casters, very nice amount of SP, and it lasted me until Life and Death dropped

The titansteel spellblade has more spellpower and crit and a minor amount of stamina but has 33 less int and a little less hit compared to the flameheart steel scalpel.

IMO, if you're looking for a starter pvp weapon between these 2, take the TS spellblade. For pve I guess it's down to personal choice.

I'm not sure why people ignore Blade of the Keening Banshee for mainhand choices.

It's pretty easy to get (1 1/2 weeks worth of dailies) and doesn't cost anything.

Compared to Titansteel Spellblade, it has -1 hit rating and about .40% less crit - which is compensated by having intellect, allowing the wearer a larger mana pool. BotKB is obviously better than Flameheart Spell Scalpel as well.
