It's for me a gamechanging trinket...at least when I am keeping up only very few people (mt or 5man healing) that don't take huge amounts of damage.

As Resto shaman with a measly 1450 healing (incl. wrath of air) and ~185 mp5 (incl. mana stream) I can:

-stay outside of the 5 second rule and fully regenerate mana while healing for ~380 noncrit as fast as the gcd allows with hw1.
-slowly regenerate mana while healing for ~760 every 2 seconds with hw3.
-stay at the break even point (mana spent/regenerated) and heal for 1.2k every 2.5 seconds with hw4.

This might not be all that helpful for mt healing in ssc and beyond. But it sure helps conserving mana, considering I can just spam anything and anyone and not only not lose mana, but most of the time continue regenerating it...

And the plenty of crits I get from talents and gear make it even better and easier to not only top off the tank (as a kind of high maintenance HoT) but keep up ancestral healing indefinitely..

Not to forget the 48mp5 the trinket amounts to with continuous max hw spamming...

And this for minimum dkp...

1st of all healing wave is no longer bugged..2nd as a raiding shaman 75% if the time u are healing the raid with spams of chainheal so this is not very effective..3rd totems can be equiped in combat so make macros..i like
/equip Totem of the Maelstrom
/cast Healing Wave X

i do this for all my casts as to maximize healing and minimize mana usage.

all in all if ure on main tank this totem is awesum but keeping a person up indefinatly with 500 Health per sec is no longer possible....bummer

The fact that this totem was not buffed in 2.4 futher shows Blizzards complete inability to do anything truely beneficial to Shamans, even if it would only be to keep in line with the other changes already made to other classes.

in 2.4 the other SSC healing relics were buffed so that holy paladins (Libram of Absolute truth) and treeform druids (Idol of the Crescent Goddess) got roughly 10% off their big heals. Totem of Maelstrom remains a measly ~3% however, not even reducing the cost as much as its elemental equivalent Totem of the Pulsing Earth, which was buffed, and now reduces the base mana cost of lightning bolt by ~9% (27 mana)

Why would they buff it? The main reason to use this is to reduce HW1 to zero mana cost (as in other ranks we would use a +healing totem)... so there' no use to increase the mana reduction (only if it was enough for rank 2, but then, it would be too overpowered)... nor increase the % healing, cause in HW1 would be no difference... besides, there are better +healing totems...
