Although you can re-acquire the Cache if you had it pre-cataclysm, you can't acquire it on a new character, and nor is it required for the E'ko Madness achievement. I was half done the achievement before I ran across the witch doctor's new location and got my Cache again. Simply killing the required creatures will randomly grant you the buff at about the same rate that you would formerly find the E'ko, and that counts for the achievement credit.

I just completed this (April 2012) and I can confirm that while Mau'ari is still present at her location, and she does indeed reference the Cache as a usable item, it doesn't affect gathering of E'ko from the creatures required for the Achievement. I tried it both with my old cache (sitting in the bank all this time) and without it. Either way I gathered E'ko from all but one of the creatures required (3 with, 3 without the Cache). What Does seem to have helped is removing self buffs. Reading the above comments, i took off my own buffs, dismissed my summoned pet, and put my trinks and enchanted weapons in my bags so they couldn't proc. Some creatures took a little longer than others, but I had all of the E'ko gathered within, I'd say 10-15 kills of each creature type (easier to do with the related quests to keep me busy).

I had and destroyed one of these. When I talked to 巫醫瑪艾里 she replaced it, even though it is no longer needed:

I lost the Cache of Mau'ari. What can I do?

Ah am very surprised that one wit such a careless nature has gotten dis far...
