This achievement is very easy to do.

The Maddening Deep is located in the south part of Azj-Kahet.

Fly to cords 67 83 and stand in the pool until you reach 100 stacks.

Boom! You got the achievement.

You can also run around in the area (I was around 62, 82) and click the Black Blood Extractors. Each gives you a stack of five.

Super easy achivement

Stand in the red pool in The Maddening Deep. This will take like 10 sec to finish.

This can also be achieved by standing in the Black Blood in the Breathing Pit while on the main story quest "Fear the Old Blood" (if you can survive the poison effect): /way 33.06 56.26

To get stacks, I clicked on the "Black Blood Extractors" which are scattered around the area. Look for the red containers.

Standing in the pools would not give me any stacks, for whatever reason.

As of recent you have to just go collect all the Black Blood Extractors. Each gives 5-10. Takes like 5-10 minutes. You can't stand in the pool anymore.
