Seems like whenever you do some Echoing fragment (with Radiant Echo) you have a slight chance that items like Echoing Fragment: Azj-Kahet drop.
You can use this item to break it, and it advance the Achievement. Completing the achievement reward the following toy: Memory Chord

To complete the Achievement, you would need to clear one Echoing Fragment in each different zone, and drop the item linked to the zone:
- For Isle of Dorn: Echoing Fragment: Isle of Dorn
- For The Ringing Deeps: Echoing Fragment: The Ringing Deeps
- For Hallowfall: Echoing Fragment: Hallowfall
- For Azj-Kahet: Echoing Fragment: Azj-Kahet

I sadly lack information on why I did drop one while doing the Worldsoul Memory in Azj-Kahet (Coordinates: 65.47 52.43) but never dropped any fragments before hand. Maybe their drop chances are low ?

Use dungeon finder to join a large group. I did x1 and it dropped 3 times! Probably unintentional though as my inventory was full and I found 3 in the mailbox.

You don't have to go to all 4 zones to use these items. I'm not sure if they're usable anywhere, but at least in Hallowfall I was able to use all 4 of the items for the achievement.

i was able to purchase all 4 of these items from the AH for 6000g total for all 4. After looting from the mailbox i right clicked each and got progress for the achi. i was not in the specified zone when i right clicked them all, but standing out in front of the AH. Right clicking all 4 gave me then achi and the toy.

Not only you can buy them from AH as HogzzTV mentioned, but you also don't need to fly to specific zones to use them.
I bought both 回音裂片：聖落之地 and 回音裂片：鳴響深淵, which I was missing, and used them in the Ringing Deeps. Both counted and I got the achievement.

There is no level requirement to use these. I was able to use them on a level 10 allied race bank alt parked in Stormwind.

If it hasn't been said yet, you can get these from the lowest level (Radiant Echo x1) version of the Worldsoul Memory.

I did the 世界之魂記憶：受傷的靈魂 in Azj-Kahet and after killing the Dregblood & Soulboil rares a couple 荊棘的回憶 appeared. This is the first time I've ever picked one up and the very next trash mob dropped 回音裂片：阿茲-卡罕特. I am not sure if it was a coincidence.

It might be coincidental, but I was not been able to get fragments while doing memories solo, while in the party I got them instantly.

I did the same memory in Ringing Deeps around 10 times (x1 and x5), no luck. While doing another x1 I notices another player in the memory, invited him, he joined and the next mob I have killed gave me the fragment.

Similar thing happened for Azj-Kahet fragment.

Now that Warbands are a thing, if you can't find an Echoing Fragment on your AH:

1. Put some gold in your Warband bank.
2. Make a new character on another server.
3. Grab your gold out of your Warband bank.
4. Buy the Fragment off of that AH and use it.

The achievement is account-wide.

You can find which servers have fragments for sale, and the lowest prices using undermine.exchange.

People say that these drop from mobs but none of the mobs are lootable? I've never received any fragments. What am I doing wrong?

If you are buying it on a cheap realm, there's a big chance you don't have a character there yet. I assume most people just create a Vulpera for the quick camp/AH access, right?

Please, DON'T FORGET TO CHECK THE MAIL. I deleted my character before opening the letter, not realizing the toy is sent by mail instead of being automatically added to the toy box via achievement.

Long story short: I've been opening tickets since January, begging Blizz for the item or to reset the achievement so I can earn it again. Learn from my mistake, it’s a nightmare.
