Mages can fall into these visions by using Blink on the metal plate between the portal to the Conservatory of Life (Freya) and the grass. It is very random if you fall through, but I have managed to do it many times by accident. When you slip through, you will fall quite a long away and will die. Make sure after a few seconds you use Ice Block or Slow Fall (Blink will probably still be on cooldown) so that you do not die from fall damage.

Once at the bottom, you should be in one of Yogg's visions. The last two times I have done it, I was in the Wyrmrest Temple vision... I'm not sure if you *always* fall into that one, I haven't tested it a lot yet. Good Luck!

REAL EASY STEP ======================(Soloed it on a retpaly)

So if u can heal yourself its no problem.

Those achs in 10 and 25 are possible in a few minutes, Switch to 10 mode then wipe 3 times, do the same thing in 25.

Steps
1) Kill adds near Sara then when she reach 1 life point she will change

2) Then get to the back of yog and wait for portals...

3) Get near a portal: 2 voices just after the portal and after (like 3 sec ):
Voice begins with : "it is done" = demon
Voice begins with : "bad news sire" = king
No voice = wait another portal spawn ( you wont die from waiting on portals)
4) Wipe and redo.

For solo 25 on this you might want to dps a vine to 20% to get to a portal ( dont get on 600 k ones), or bubble to portal.

WOD UPDATE

No need to die after each vision - just wait around for more portals to spawn and keep entering. I got 3 / 3 different visions, perhaps was lucky, or the portals spawn in a cycle no sure. Either way, don't need to keep dying.

Just did this and can confirm it can be done in one try. Also, it seems the visions are rotating in order, at least on my try. Clear the rooms in the visions, run past the brain through the portal, get out and wait for next portals. Repeat a few times and achievement is yours! On the last vision you need, don't run past the brain but kill it, then kill Yogg-Saron once you are out. Did this as a level 100.

How is it that when I try this at level 100, I continue to get charmed or otherwise incapacitated while I'm waiting for the portals. I'm sure it's a simple answer, but I don't see anything here addressing it. and Also when I did enter one time, there seemed to be a force field preventing me from entering the brain room.
