make sure you are specced into Pain supression, then make sure to use it often, I would pop it at the beggining of the first phase so that it will usually be up again by phase three. once it does, cast it on the tank and allow yourself to almost "forget the tank" after you cast it.
most people dont recognize that you should bubble yourself before Divine hymn or your first PoH to get haste buff AND reduce push back....you might not think it, but push back will lose a tick or so on Divine hymn in phase 3.
Then comes the sad news....damage is coming too quick to do a PWS->PoH->PWS->PoH->...rotation for that phase. instead save Power infusion for YOURSELF, and then jsut PoH spam until you are out of mana....
in review
P3 starts:
PS tank
PWS self
Inner Focus (forgot to mention this DA procing gem)
PoH
Power infusion self
PoH
PoH...
(2 group members at 1/2 health)
PWS self(or other if still have Weakend Soul)
Divine Hymn
PoH
PoH....
I know , PS wore off before you cast the second PoH is even cast, but it doesnt matter. Phase 3 is group damage almost exclusively....heal accordingly (or in other words, act holy for a minute :p)

Well, for my disc spec, phase 1 and 2 aren't really a problem since there isn't much AOE damage. The dps should know to not pull aggro during them.
During the 3rd phase, I'd just have everyone stack up with the tank, having pre-shielded everyone. First, I blow Inner Focus + Divine Hymn. Then I just use Power Infusion and spam Holy Nova.
I don't use PoH as much, as I find that abysmally slow as Disc, even with the PW:S buff.
Seems to work for me, as I've never failed H ToC as disc yet. :)
ETA
: You can shackle the ghoul during the first phase, so your group doesn't have to worry about it.

I always see the biggest problem during P3 of the BK fight to be the Marked for Death debuff. Make sure you bubble that player and keep a PoM dinging around. Also use your PoH hasted and FH those who really need it.
Remember, if you cast your Shadow Prot buff then that's not going to cut the damage because his P3 Damage is
Shadow/Frost
. I've found that using any Frost Resistance (totem, pally aura, whatever) in combination will cut your damage by a HUGE amount. With both resistances high, you should rock the 3rd phase and not even realize that much damage was going out. I think it cuts the damage taken by 50%.
Also, remember to keep your Abolish Disease rolling on your tank during P1 and P2. It'll help keep tensions low by the time you reach P3.

Spamming
Prayer of Healing
with
Enlightenment
,
Borrowed Time
and
Power Infusion
is way faster than casting it with 3x
Serendipity
... Basically 51% haste as Discipline vs 36% as Holy... And we don't have to spam FHs to build up that speed.
Actually, not really accurate as well, since Borrowed Time only lasts for 1 spell.
Your first PoH will get the 51% haste bonus, but your succeeding PoHs will only get 26%. Casting more PW:S will cost you the GCD, hence not really making your POH that much faster. :)

Just a quick note, I found for phase 3 I was spamming Binding Heal and that seemed to get me through the stress of all the damage put out. I'm not sure if it's the best way to do it - we did lose two of our DK's but it got the job done!

Had this happen to me on my resto shaman 2 days ago because a DPS was doing about 1.5k DPS while the other 2 did 3k which wasn't enough to offset the low DPS person.
Wait, you got 1.5k + 3k + 3k and you blame the dps? WTF?
Yes, the 1.5k guy clearly wasn't pulling his weight but 7.5k combined dps (plus anything the tank did) is more then enough for that fight. You should have been able to heal through that.
A few days ago I successfully healed it (on my Druid, but that shouldn't really matter since Priests are just as capable) with only 2 dps (the 3rd was standing in the ghoul explosions and instantly died after p2) and each did less then 3k. The dps race really isn't that bad.
The only thing that usually leads to problems is if they all catch an explosion or two in the transition between p2 and p3. If they do they will start p3 on half health and that usually leadw to a wipe.

BK Phase 3 easymode:
Shield Self >> PoH >> Shield Tank >> PoH
Basically repeat this until everyone has weakened soul or folks are getting low. I rarely need to pain suppression myself unless the BK decides I'm marked for death. But don't be afraid to use PS if you think it's needed.
After the first few PoH's, Inner Focus >> Divine Hymn. Then PI self >> PoH spam, stopping at this point only to shield anyone that's getting lower than you're comfortable with. BK should die in the middle of PoH cast.
Also helps to get frost res + shadow res auras up.
All in all, I've never wiped healing this fight, to me it was the easiest fight to manage.
