This quest is quite simple, but you must remember, after picking up this quest, STAY NEAR HIM. He creates the torch within a minute, and will send a message saying "Please be patient, <name>" Some people walk away because they don't realize it's a waiting game.

After he's all done, pick up The Torch of Retribution part two.

I accepted the quest but there is no torch in my inventory. Where do I find it?

Torch is laying on the ground in front of the guy who makes it.

You have to light the 4 towers, north, east,south, and west, the towers are on the east side of the cliffs of the cauldron,
if you have discovered most of the area they will show on the map.

After you get the torch from Kalaran Windblade you have to speak with his squire before you can light the torches....be sure to click on the torch after Kalaran makes it.

You can definitely fail this quest. I will neither confirm nor deny that I have done this personally, but I will say that if the quest NPC disappears (for instance, when someone else hands in a future quest in the chain) during the creation process, you will automatically fail the quest. Also, incidentally, the NPC takes roughly 5-10 minutes to respawn. Or at least, that's how long my guild spent laughing at me for failing this quest.
