In my experience, that happens when you have just completed an objective and QH hasn't yet updated its map icons to match your progress. If you're quick enough opening the map after turning in a quest, you can always catch it there. :D

I love the message you get when you get odd flightpaths like the one on the floating island (for alliance) in hellfire. QH has a great sense of humor
