some "patch notes" are actually mmo champion doing a comparison and noting the differences (they usually title these 'undocumented changes'. some are officially released patch notes which may or may not actually be current to the patch on the ptr (blizz have several builds at different stages of the testing process).
the 5% human error debuff is applied to all of these ^^
if in doubt log in to the ptr and check but most of these are easy to figure out. the calculators here and on mmo champion are direct versions of the in-game ptr calculators and are not made by applying the patch notes as they read. as such they are probably as close as you'd get to 'definitive' if you don't want to or can't log into the ptr yourself.

erm... that's what patch testing is for. report them to blizz not here.
