First Steps
Opening Rank 3 of the Mirror Network initiates the quest,

teaches you how to repair broken mirrors using a repair kit. After that, you are on your own.
The first thing you need to do is visit

in Sinfall. She is hanging out with

, and is not a vendor, but once you are eligible she will sell you a

for . Then you need to find a Broken Mirror to repair, walk through, and profit.
There appear to be 3 Broken Mirrors which lead to chests you can loot once a day.
The Broken Mirror locations change every weekly reset.
The drop rate for transmog pieces seems fairly low.
Speculative: The 3 Broken Mirror locations might differ on select server regions, but have proven to be consistent between EU (English) and US realms.

At this point we don't know exactly how many Broken Mirrors there are. This may be all of them, or there may be more. We also don't know whether they always show up in the same group, or whether the mix and match. Check the comments on

Tips
Raapnaap also notes that there is a bug. Occasionally passing through your repaired Mirror will take you to the Sanctuary of the Mad instead of your correct Mirror destination. If this happens to you. don't panic, they have the solution.
The first time you go through a Broken Mirror, you get a buff called
Visitor Aura
. Do not remove this buff. Instead, go through the Broken Mirror again, and this time it will send you to the chest room.
Another commenter, jdevlin7756, offers this tip regarding the initial quest,

If, like me, your Infused Rubies are maxed out at 100, accept the free Handcrafted Mirror Repair kit and then buy another. It will cost 20 Infused Rubies which you will recoup during this quest. Otherwise you are just throwing away the Rubies.
Rewards
There are many worthwhile rewards for taking the trip through the Broken Mirrors. Most notable are the transmog set, which is a splendid recolor of the Venthyr Covenant set, along with a gorgeously daggery matching back piece, a pet, and a mount.
