If you're curious as to how this item scales down with your character's level (for purposes of twinking or general curiosity), you can use the LINK button at the top of this item page to link the item in-game: the WoW client will automatically scale the item stats down to your character's level for you. This works with all the other Heirloom items as well.

Steps in case you are unfamiliar with how to use Wowhead item links:

Click the link button above, there will be a popup window with some automatically highlighted text.
Copy the text (Windows: Ctrl-C, Mac: Cmd-C) and close the popup window.
Open (or switch to) WoW and press Enter to open up your chat dialog.
Paste the code into the box (Windows: Ctrl-V, Mac: Cmd-V) and press Enter.
A message in white text will appear in your chat log followed by an item link.
You can click this link to view the details and can Ctrl-click it to view it on your character.

Don't forget that you can also spend shards on Wintergrasp Commendations which can be used to buy:

Insignia of the Horde (2805 honor / 60 shards) which offers the same use but no resilience
Insignia of the Horde (8k honor / 120 shards) which offers a 2 minute cooldown and a fixed amount of resilience.

Unless you're trading items between multiple twinks, it would seem that simply going the honor->bought item route would be more efficient.

This item is ONLY useful if you are under Level 58 and attempting to create a 'twink' character. The Alliance racial is still considered quite overpowered in the sub-61 BG brackets by many people, so this would be a nice way to somewhat even out the score and add some resiliance to boot. Resiliance at that level is incredibly powerful and provides a hefty benefit the lower you are in level.

Utterly worthless item for anyone Level 58 or above.
Just get a portal to Shattrath and head to Outland to Hellfire Peninsula.
Then head to Thrallmar and do the "Hellfire Fortifications" (http://www.wowhead.com/?quest=13409) quest.

To do that quest requires a pre-quest chain that begins with 'Warlord Dar'toon' (http://www.wowhead.com/?npc=19254).
Oddly, some of the quests in that chain (as well as Hellfire Fortifications) are said by WoWhead to be available at 55, yet you can't even obtain any of them till 58 due to the pre-quests.

So unless you're making a twink for the sub-51 BG brackets, save your shards for better things.

Unfortunately, this does not trigger the achievement Freedom of the Horde nor, respectively Freedom of the Alliance.

Tested it on my alt yesterday.

Be wary that though you can send BoA items to characters on the same realm but of opposing faction since 3.3.0, this item can not be used by Alliance toons. It does not transform magically into an Inherited Insignia of the Alliance.

Had to find this out the hard way at the cost of 250 Stonekeeper Shards.

As mentioned by Garon132 in reply to this post, this issue is most likely fixed in patch 4.1.

I bought this yesterday and sent it to my aliance alt. It didn't switch to aliance version, so I ticketed GM.... after several hours of waiting as usual he told me he won't switch it. Maybe it depends only on their will and they don't have any instructions about it.

Pointless heirloom, really. As noted above they don't seem to like being mailed cross faction, the resil is negligible, and the cooldown is 5 minutes. All for the low price of over 2k honor. Just grab the regular Insignia of the Horde/Alliance from the Legacy Armor Quartermaster for 55 honor. No level requirement either, and aside from the laughable resilience it's the same item.

I bought the Alliance version and sent it to my Horde toon without reading here 1st. When I discovered my Horde toon couldn't use it I opened up a ticket asking for a refund or to change the item into the Horde one.

The GM changed it to a Horde one and put it in my mailbox.

As of Patch 4.2 this item now switches between Alliance and Horde versions when sent across faction

TWINKS REJOICE!

In patch 5.3, the trinket will FINALLY have a CD of 2 mins down from 5!

This means that it'll have 24 more resi than the blue one, equaling 3% resi at 70, WITHOUT having 3 mins extra cd.

With the new heirloom upgrade system introduced in patch 6.1 this item has been replaced by Унаследованный знак различия Орды.

Zam/Wowhead's comment in red that says "This item's source is no longer available/removed." is incorrect. Today, March 6, 2017, I just used my horde toon and bought the Inherited Insignia of the Horde from Estelle Gendry, the Heirloom "Curator" who is now located at 78, 76 in the Rogue's Quarter in Undercity. This is in Legion, Version 7.1.5 (23420) last updated Jan 12, 2017. I now have it on both of my WoW accounts.
