Curious how this "stacks" to 5 when the CD is 1 minute, but the debuff duration is 10 seconds.

This persists indefinitely in combat as buff, rather than on the target.
When you drop combat, it becomes a 15 second buff with your current stacks.
If you re-enter combat it becomes permanent again, and it can be refreshed by reusing it.
Though the trinket is likely not amazing in M+ purely for AoE, it's a good funnel trinket. If you primarily use it for funneling, it's better to use the trinket to extend the buff rather than to use the AoE blast which is very minor.
The "Use" ability doesn't incur the trinket cooldown, nor is it affected by the trinket cooldown.
I've been running this for quite a while as a paladin tank now in M+ and the damage it provides is massive. Almost all of the damage is from the passive application via Blessed Hammer, so I'd recommend trying to save the use effect to maintain your stacks between pulls once you've stacked it to 5.
If you time it you can even let the healer fully drink without losing the stacks most of the time. The AoE when used is pretty tiny.

The anvil empowers Heart Strike, which can hit multiple tagets while standing inside your Death and Decay.
After a little experimentation it appears that:
- when the anvil procs, the extra damage is applied to only one target, BUT
- when hitting multiple targets, it is possible to get multiple procs with the same hit before triggering the internal cooldown
In other words, supposing you are hitting the maximum number of targets (5) with your Heart Strike, your procs will sometimes affect more than one target, with decreasing chances up to 5.
If your timer is running out inbetween pulls in M+, try saving a use cooldown or attacking a critter to reset the 15 second buff before you lose all your stacks.
I tested a 441-ilevel version of this trinket with a Mythic+21
run. As a tank, the

on-use effect CAN crit and is an AoE that will hit multiple melee targets in range. The cast time of 0.5 seconds does scale with haste.
passive ability can also crit and this crit chance is independent of whether or not your triggering ability was a critical strike.

damage scales with versatility.
For brewmaster, the Press the Advantage talent that turns Tiger Palm into a passive (choice node with weapons of order) Pocket Anvil will deal no passive damage since PtA doesn't count as tiger palm damage.
If you do have tiger palm still, it's fine.
Simmed this trinket out when it dropped in raid finder. Simmed it at 424 and it was a ~1200dps down grade to Trampling Hooves 408, Spiteful Storm 411 that I had on. A bit disappointing for Fury warrior I guess, imagine if I spent the time to upgraded it to 424.
My 441 version of this trinket as a Fury warrior on dummies is only adding 1.6% DPS - which seems wildly low, I had been using a 411 Drogbar Rocks which was consistently doing about 5% DPS
As a Mythic Raid Guardian Druid, I love this trinket. Could care less about the dmg numbers, as soon as a group in combat hears that "DING" on use, it is a clarion call to crank it up. Almost everyone in the group starts blowing DPS and Protection cool-down's. That "DING" is the WoW call to battle. Another awesome trinket Blizz.
