I'm pretty sure he's just a lich, who, since he was allied with the Death Knights, broke free from the Lich King's will and happens to train death knights. I don't think he has any lore behind him. The name is just a common structure for liches.

I think the opposite is true - Alam'Thazad decided to change his name to something more scary upon becoming an Arch-Lich so that he could be just as well-respected as Kel'Thuzad.
Kel'Thuzad set the naming fad, and now all the Arch-Liches are doing it!

Amal'thazad is just what they choose to be the Frost Trainer for Death Knights, guess they figured cause Kel and all the other lichs love the frost spells so much, they would use that as well.
