To complete the quest you need to find a suitable weapon for Thrall.
There are some Mawsworn Armaments - (5) within the area you can search for the axe.
I'm not 100% sure you have to check every one before you get the axe, but every time I had to check out all 5.
Dont forget to pick up Wounds Beyond Flesh it can be completed along.

I checked all the racks, they disappeared off of my minimap but the quest was not complete. I just abandoned it, re-received it from Thrall, and was good to go

I present introduction to Shadowlands quest chain. You have to do 22 quests after finish Stand as One you will achieve Into the Maw.

Quest Name:	Start and description
1. Shadowlands: A Chilling Summons	Speak with High Inquisitor Whitemane to see video then follow Highlord Darion Mograine He will open portal. Then speak with Highlord Bolvar Fordragon to finish.
2. Through the Shattered Sky	Move 4 shards to them place to finish Ritual. You will be teleported to The Maw
3. A Fractured Blade	You need to rallied 10 Death Knight. You doing this quest together with 4. Mawsworn Menace
4. Mawsworn Menace	Kill 8 any Mawsworn forces. finish quest to Highlord Darion Mograine He will run with You.
5. Ruiner's End	Start this quest from Wilona Thorne 23 52. Kill Ruiner Maroth 20 46 and move to 21.5 42.
6. Fear to Tread	You need to find 3 clue - 22 43, 21 39 and 21.5 36 then find Lady Jaina Proudmoore 22 33.
7. On Blackened Wings	You will see video then kill 6 Mawsworn Harbinger.
8. A Flight from Darkness	Escort Lady Jaina Proudmoore to the cave.
9. A Moment's Respite	Speak to Jaina
10. Field Seance	Use Shattered Helm of Domination on 5 Malice Shadow when they are under 50% hp.
11. Speaking to the Dead	Use Shattered Helm of Domination on Tormented Amalgamation 28 31.5 when He is under 20% hp.
12. Soul in Hand	Take Soul to the cave.
13. The Lion's Cage	Go to Upper Tremaculum use portal under it 27.5 17.5 and find Anduin Wrynn.
14. The Afflictor's Key	find Afflictor's Key is droping from Phael the Afflictor 28.5 11.5 100% drop. You can do this quest together with An Undeserved Fate.
15. An Undeserved Fate	find Mawsworn Key killinf mobs around then open cages to released 15 Souls. Move back to Anduin Wrynn.
16. From the Mouths of Madness	You will be teleported then speak with Highlord Darion Mograine.
17. By and Down the River	Go to River of Souls 29.5 36 ( is on bridge ) then will be a movie after move to 29.7 42.5 will be one more movie
18. A Good Axe	Find a suitable weapon for Thrall in Mawsworn Armaments you can do this quest together with Wounds Beyond Flesh.
19. Wounds Beyond Flesh	Fine Wither Blade is droping from Mawsworn Soulrender
20. Draw Out the Darkness	Kill Withering Presence.
21. The Path to Salvation	Follow Lady Jaina Proudmoore
22. Stand as One	Use and defend Eroded Waystone when will get to 80% use Eroded Waystone one more time will be video and chain fill finish :)

If you are too fast and pick up the next weapon before Thrall finished his complains with the last one the new weapon is not properly counted. I had to abandon the quest and restart because of this.

In case you want to read it altogether, here's the dialogue that occurs during this quest with significant actions in < >.

<Upon searching the first weapon rack, Thrall finds a crossbow.>
Thrall: A crossbow? I prefer to strike my enemies face to face. I suppose it's got heft to it. Let's see what else we can find, though.

<Upon searching the second weapon rack, Thrall finds a staff.>
Thrall: I've never been one for staves, but at least it won't break in a melee. Let's keep looking.

<Upon searching the third weapon rack, Thrall finds a two-handed sword.>
Thrall: A serviceable weapon. Though I've never understood why the Alliance seems so fixated with swords. Too much blade, not enough haft.

<Upon searching the fourth weapon rack, Thrall finds a polearm.>
Thrall: Hmph. Too long a handle. Better than nothing, I suppose. Let's take one more look.

This is my 6th-ish time through the Maw and I JUST noticed that Thrall is literally wailing on things with a loaded crossbow.

Description

I was a damned fool for throwing my axe at that sea hag. We can't afford to lose focus. The battle ahead won't be easy. I'll need another weapon to help us fight to the waystone. Looks like these soldiers have plenty of armaments. Let's keep an eye out for one I can use.
