To complete the quest you need to cross the bridge over Gorgoa, the River of Souls.
As usually, follow Jaina and pals towards the brdige over Gorgoa and make an attempt to cross it
Unfortunatelly there Helya appears and push you off the bridge into the river of souls.
Run to the marked spot and leave the river so you can turn in the quest

Something about being in the river of souls is one of the more chilling, horrifying moments I've ever experienced in all my years playing this game. The ambient sound that plays is suitably unsettling, and the sea of the dead rushing past you one after another, all speaking in a clamor, paints a grim, doomy image. Here's a collection of quotes I saw from the souls of the damned:

While there are some amusing ones in the mix, the majority are a stark insight into the final living moments of some of these souls. The most interesting, to me, is the soul speaking Shath'Yar, the language of the Old Gods. Perhaps those are the souls of Shadow Priests who succumbed to the persistent madness at the fringes of their minds, or perhaps it's something a bit more sinister. We may never know. But this is already, in my mind, one of the best mood-setting, world-building, emotion-provoking bits of questing I've ever done, in spite of, or perhaps thanks to, its simple execution.

I present introduction to Shadowlands quest chain. You have to do 22 quests after finish Stand as One you will achieve Into the Maw.

Quest Name:	Start and description
1. Shadowlands: A Chilling Summons	Speak with High Inquisitor Whitemane to see video then follow Highlord Darion Mograine He will open portal. Then speak with Highlord Bolvar Fordragon to finish.
2. Through the Shattered Sky	Move 4 shards to them place to finish Ritual. You will be teleported to The Maw
3. A Fractured Blade	You need to rallied 10 Death Knight. You doing this quest together with 4. Mawsworn Menace
4. Mawsworn Menace	Kill 8 any Mawsworn forces. finish quest to Highlord Darion Mograine He will run with You.
5. Ruiner's End	Start this quest from Wilona Thorne 23 52. Kill Ruiner Maroth 20 46 and move to 21.5 42.
6. Fear to Tread	You need to find 3 clue - 22 43, 21 39 and 21.5 36 then find Lady Jaina Proudmoore 22 33.
7. On Blackened Wings	You will see video then kill 6 Mawsworn Harbinger.
8. A Flight from Darkness	Escort Lady Jaina Proudmoore to the cave.
9. A Moment's Respite	Speak to Jaina
10. Field Seance	Use Shattered Helm of Domination on 5 Malice Shadow when they are under 50% hp.
11. Speaking to the Dead	Use Shattered Helm of Domination on Tormented Amalgamation 28 31.5 when He is under 20% hp.
12. Soul in Hand	Take Soul to the cave.
13. The Lion's Cage	Go to Upper Tremaculum use portal under it 27.5 17.5 and find Anduin Wrynn.
14. The Afflictor's Key	find Afflictor's Key is droping from Phael the Afflictor 28.5 11.5 100% drop. You can do this quest together with An Undeserved Fate.
15. An Undeserved Fate	find Mawsworn Key killinf mobs around then open cages to released 15 Souls. Move back to Anduin Wrynn.
16. From the Mouths of Madness	You will be teleported then speak with Highlord Darion Mograine.
17. By and Down the River	Go to River of Souls 29.5 36 ( is on bridge ) then will be a movie after move to 29.7 42.5 will be one more movie
18. A Good Axe	Find a suitable weapon for Thrall in Mawsworn Armaments you can do this quest together with Wounds Beyond Flesh.
19. Wounds Beyond Flesh	Fine Wither Blade is droping from Mawsworn Soulrender
20. Draw Out the Darkness	Kill Withering Presence.
21. The Path to Salvation	Follow Lady Jaina Proudmoore
22. Stand as One	Use and defend Eroded Waystone when will get to 80% use Eroded Waystone one more time will be video and chain fill finish :)

If you cross the River of Souls without trying the bridge first, you won't get credit. You need to head to the bridge, try to cross it, and then get knocked into the River.

In case you want to read it altogether, here's the dialogue that occurs during this quest with significant actions in < >.

<Upon exiting Forlorn Respite, this short conversation happens.>
Jaina: You're troubled, Thrall. What is it?
Thrall: <He gives a heavy sigh.> Still no sign of Tyrande. We cannot leave her alone in this nightmare.
Jaina: Agreed, but we have no idea how to find her. We will do our best, but this waystone may be our only means of escape. If there's a way out of the Maw, then there has to be a way back in. We will return for her.

<Your group, consisting of Jaina, Thrall, and Anduin, continue towards Gorgoa, the River of Souls. Halfway across the bridge, dark tentacles block the bridge on both sides. Helya comes up from the River.>
Thrall: Hold. I hear something stirring...
Helya: Your wretched souls will feed the coming darkness!
Jaina: "Wretched"? I heard you were soundly beaten by a force of "wretched" champions, witch!
Helya: You will pay for that insult! The skies of your world will be darkened by the wings of my Mawsworn!
Thrall: For Azeroth!
<Thrall throws his axe at Helya, angering her further.>
Helya: Drown! Drown among the souls of the damned!
<Helya knocks the whole group back and off the bridge. In the River, souls of the damned fly past you, moaning and groaning.>
Jaina: (yelling) What is this madness?
Thrall: (yelling) This sound... it's unbearable!
Anduin: (yelling) Keep moving!

<Your group continues until the slope becomes climbable on the other side. You run ahead, exiting the River of Souls.>
Thrall: Look! On the precipice above! Is that...
Jaina: The Jailer.
Anduin: He has Baine!

<Upon turning in the quest, a short cinematic starts.>
The Jailer: Pitiful mortal. I find your spirit...
<Baine Bloodhoof groans.>
The Jailer: Unworthy.
<He throws Baine from the rampart he is standing at.>
Thrall: Baine! No!
<Fading back to the group, Anduin is attempting to heal Baine.>
Anduin: His wounds are mending, but his spirit remains weakened. I've never seen anything like it.
Thrall: I believe I have. In a place very far from here...

This quest is available for both factions, despite the faction tags on it. The other one is an unrelated quest in BFA.

By the way, I'm glad the text says Zovaal's is across the river, because my Maw zone map was bugged and seems to think the Cauldron/forge extended across the river to the Plains of Torment. So at least the quest text confirms that was a bug!

Welp. I did it. I seared a twisted image onto my brain. After you get knocked off the bridge you can go back towards Helya and look at her "legs".

I don't know why I was expecting her to have legs. But those definitely weren't legs.

For the first step (before meeting Helya) you can't just go down into the river of souls and run across it; you have to go across the bridge. If you get lost running around in the river like I did, get back out on the north side and go up the hill which starts at /way 29, 34 .

Tip : You don't have to wait until they finish talking, just jump down the bridge and you will get the credit for the quest. When you see the tentacles spawn you are safe to jump.

Description

Our next step is clear. We must travel to Zovaal's Cauldron--a frightful place I've heard spoken of in fanatical whispers. It's a forge of sorts that lies across the River of Souls. I have only been there a handful of times, but I have seen Baine held captive by the soldiers there. If fortune favors us, he'll still be nearby. Once he's safe, we can see about finding this waystone the shade spoke of.
