The quest NPC can be found at the back of the cave (The Skull Warren entrance).

Keep left when entering and, of course, you need the disguise on. ;)

The cave is named "The Skull Warren" and it's located half way up the mountain, on the RIGHT side.

Just follow the cave to the end and you'll find him.
It's filled with Blackrock Necromancers.

don't be fooled by the map icon, these comments are accurate, with the xpac (cata) they moved some of the icons and as of this patch the in game map is inaccurate.

Neeralak is located at 70 47 inside the Skull Warren. Cave is off one of the ramps leading to Dreadmaul Rock. Once you enter the cave, just stay left

I had the disguise on, but when I found him he was red not green. I swam down into the bottom of the water, took my disguise off then put it back on. When I surfaced he was green and then I could complete the quest.

The cave your looking for, you'll spot some Blackrock Necromancers patrolling outside and inside. It is almost directly on the other side from the Firegut furnace cave with the ogres.

Ok, the tricky part to this quest is finding the cave, when looking on the map it does not show the entrance. On the northwest of the hill(Dreadmaul Rock) you will see a Rope and Wood Bridge, just the left of that bridge is trail that goes up and you will cross over a dirt like bridge with horns sticking out on both sides, which will lead you to the cave. Once your in the cave stay to the left the whole way down the bottom of the cave and you will find your guy.

The question mark on my map was at the wrong place.
The cave entrance is at 71.4,37,9.
The guy is on the bottom of the cave.

As per wrocamb's post, you have to cross the bridge. The bridge starts at 67.28, 35.09. the cave entrance is framed by grey rocks at 71.14, 37.98.

Confirmed that the question mark was directing me to the wrong spot. The entrance to the cave (for those that do not have coordinates) is on the side of Dreadmaul Rock. When looking at the map it is slightly north of the L in the word Dreadmaul exactly in the red fire line.

Hi Guys
So after While i back to Burning steppes to complete my story line from this zone i was on 3/4 and now i'm level 120

so there is problem i need Blackrock Disguise and this is for while ago cant remember what happened to my quest item

so i read so many comment and didn't get answer so try to tell what happen to me!

first of all if you need this quest item to complete your quest go

find Thelaron Direneedle and speak again for each race its different you can find in wowhead your race tip
note:
if you try to get this as a allied race like :dark iron dwarf : check Dwarf part
or lighforge check Draenei
highmountain tauren check tauren and so on .....

so and again there is nothing for me the npc wont speak whit me and do nothing !

So i back to my messy bank I click on search button and search the word Blackrock disguise and i see its there on my back and that's why i cant speak to npc again for new one

this quest item need for some quest like

The sadistic, incompetent designers of this mountain should be fired immediately. The useless map leads you to the wrong direction too. It should not take me forever to TURN IN A damned quest.

As someone else mentioned in the comments, the cave entrance is 71.4,37,9. ( I use Tomtom add-on)
There are two BlackRock necromancers in this cave go inside, the guy you need for the quest is way in the back Dragon-Lord Neeralak inside the cave. Good spot for ore too.

A Delivery for Neeralak

Using your Blackrock Disguise, sneak into the Skull Warren and convince Dragon-Lord Neeralak to take your Crate of Sleeping Flamekin. Speak with Colonel Troteman if you need a new Blackrock Disguise.

Description

The warlocks of the Blackrock Clan often practice their magic in the Skull Warren, a cave on the eastern side of Dreadmaul Rock. Their leader, Xi'lun, is busy elsewhere, so the cave is currently being held by another of the big three: Dragon-Lord Neeralak. And Neeralak's expertise is in dragons, not in necromancy.

I've used the slumber sand on those flamekin you captured. Try to convince Neeralak that they're warlock imps, and not flamekin. When they wake up, they won't be happy.
