Sigh. So before I get to the cave, someone else releases Anya from the cage and Sasha spawns to offer the next part of the chain. I arrive and turn this quest in. Then as I'm reading the quest text for Hour of the Worg, they both despawn. About a minute later, Anya's back in the cage but I have no way of getting Sasha to appear again because she only appears after someone turns this quest in.

If you miss Sasha's spawn here, all you have to do is go to her normal spawn at White Pine Trading Post (57.4,41.4).

She speaks as though she'll help you find the cave... but when I accepted the quest she didn't start walking or anything. Bug or working as Intended?

Yeap, she's in the cave, don't let the questhelper confuse you in this one either.

Be careful here. Someone talked to the little girl ahead of me, and so they were outside the cage. I completed the part of the quest with her, but then the other person with the quest marker above them disappeared before I could accept the next part. I had to wait until someone else came along and did it.

if you found here her sasha run in and they have a litle talk.

Anya says: You won't eat me, will you?
Sasha says: Anya! You're all right!
Anya says: Sister! The mean men were going to take me to Badmoon Isle. They wanted to turn me into one of them.
Sasha says: Badmoon? You mean Bloodmoon?
Anya says: That's what I said! Badmoon! Sasha... don't ever leave me again! First they took papa, then they took you... I don't want to be alone with them!
Sasha says: Don't worry sister... no one can hurt you now.

Respawn in the cave is pretty quick, I was doing this with another person, I clicked Anya first, did the whole talk, got the next quest, we waited about a minute for her to spawn again for the other person.

You must first aquire Mikhails Journal and complete the quest before you can do any of the quests involving Ruuna the Blind or Sasha OR her little sister.

I spent an hour finding this out the hard way :/

Pretty surprised no one got the obvious D2 reference here. In act 5, you go rescue Anya from nihlathak...in a cave no less.

Anya and Sasha, two sisters, and a lot of wolfs going about.
This isn't a reference to D2, it's a reference to a song of ice and fire. Sansa and Arya of house stark, whose house sigil is a direwolf.

Anya can be found in a cave, sitting in a cage. Entrance is at 65.3 43.3.
