Anyone know the location for thran'tiok? even wowhead doesn't seem to know.

For this quest you need to create the items from Dactylis @ Elysion Hold.
Go farm 5x (New)Soul Mirror Shards at 59:59 in Bastion, 15 Shards if you are planning to do all three quests.
Once you have Created the Soulseeker Crystal and a new Soul Mirror you can hand in the quest and continue onto Exiled by Paranoia.

If you have a "Soul Mirror" in your bags it might be from a previous questline, the "Soul Mirror" you want should be white text with no 'use' option (I had one in my bags and was wondering why it would not let me craft).

As a prerequisite for this quest you will need to unlock tier 2 of the Path of Ascension. I also had to complete the Kalisthene Loyalty battle.

If the quest is available at your Path of Ascension area and you've made the soul mirror and lure, but still for some reason can't continue the quest, you may need to delete the soul mirror in your bag and recraft it. As Purpleurza recommended, unlock tier 2 and beat Kali Loyalty first before trying this just to make sure that's what you need to do since it can be a pain to craft.

These quests are so weird and unintuitive!

You can't actually accept the quest until you craft the items. And the items you craft have weird constraints (for instance you can't craft a Soul Mirror if you already have one in your bags and weirdly sometimes they don't count if you have one in your bags).

Once you are finally able to craft these items you can finish this quest, which will give you the "real" quest to go use the Mirror.

This quest unlocked for me after having completed my first Trial of Loyalty.

If you're stuck in PoA and can't get the last two fights (8/10), you may be like me and somehow dropped or bugged out the questline. Check if you need to pick up the quest Filching Phylacteries to continue.
