For this one, you need to kill the boss while everyone in the raid is fixated by a ghost. On Normal, all of the Ghosts from P2 despawn when you transition into P3, so you need to not kill the boss until theres a ghost on every player. (We assumed that this also means that everyone needs to be alive, but I cant confirm for sure.)
Theres 2 Ghosts spawning after you break out someone Mind Controlled by Dreadmarch, which is cast by Malacrass, which then fixate on random players.
You cannot have these ghosts get destroyed by the tank frontal, which is how you are usually supposed to play the mechanic. Kite them into a corner by looking away from them, and then look at it to freeze it in place.
With 15 People in the raid, the P3 Dreadmarches went out on 3, then 4 people (i think), making us have 14 ghosts. So we had to wait for 3rd cast in P3, which is quite a while. With less people maybe you only need 2 casts.
As soon as everyone has the debuff, the achievement turns white and you can kill the boss.

On Heroic, if the ghost that fixates you reaches you, you get MC'd again and spawn another 2 ghosts, which would make it way faster to complete, also the ghosts in P2 dont despawn iirc, but on normal the ghost just knocks you away, potentially knocking you off the platform, so be careful.
