Three of the mobs spawns as the objective to strike the Underbelly while the Szarith the Fanged is a rare spawn who might not always be up and Vserix the Sneaky is sneaky sneaky so I'm not sure if he's up every event and hiding or another rare spawn.

This causes trouble because apparently progress on this achievement is resetting every time the strike finishes without successfully killing all five in one go so if you're going for this achievement right now make sure you get lucky with the rare and tag them all!

EDIT: I managed to get a Vserix and Szarith kill while the Temple Strike was not active and credit for them persists however when the Temple Strike begins and when it ends I lose all credit for Priest of the First Rattle, Champion of the Scale and Guardian of the Sacrifice so it's a good idea to get Vserix and Szarith beforehand because you need to be there and ready to hit all three Temple Strike objective mobs if you want to finish this achievement. It's such a pain. :(

This achievement is completely bugged right now. Every time you complete the Underbelly event, one of a few things happens:

Progress on Priest of the First Rattle, Champion of the Scale or Guardian of the Sacrifice is kept.
Progress on Vserix the Sneaky or Szarith the Fanged is kept, but the rest gets wiped.
You lose all progress on the achievement.

Frustrating, yep, but until it's fixed the only viable option seems to be getting all 5 kills in the same run. Not totally sure about Vserix's spawn conditions though... I currently have everything reset except Vserix, so I'm going to try killing the other 4 in one run and hope it works.

EDIT: Got the achievement! Managed to see all 5 NPCs in the same run.

Champion of the Scale is always found in the first room, so keep an eye out cause if you're running with a big enough group, they might kill him before you even get a hit in.

Once he's dead and you continue through the rooms, right before the stairs leading up to the room with Priest of the First Rattle, I found Szarith the Fanged. Every time it's spawned for me during the event it's been in that room, and if you have nameplates enabled it's easy to spot with a bit of attention. Just hit it once and pull it into the corridor so the group finishes it off with AoE.

Once you go up the stairs you'll reach the room with Priest of the First Rattle. IMPORTANT: I'm not 100% sure if Vserix the Sneaky spawns in that room or in the next room to the left, before you reach Guardian of the Sacrifice (which is in the room to the right after that), look for Vserix there. In my run a Paladin moved to the back of the priest's room and pulled vserix, and by the time I noticed it was already in the middle of the room, so I couldn't get a clear look at exactly where it spawns.

Finally, kill Guardian of the Sacrifice, who's always in the same room marked by the Temple Strike quest, and you should get the achievement just like I did.

This achievement is bugged right now. Make sure you have credit for Vserix the Sneaky & Szarith the Fanged before trying for the ones you get from the strike to be able to get this achievement. The strike ones will re-set if you don't. I was able to get the 3 strike ones in one go but assume it will also reset if you don't get all 3 (after already having the other 2 complete).

Can confirm that you keep credit for killing Szarith and Vserix across Underbelly strikes, but the other three (Priest of the First Rattle, Champion of the Scale and Guardian of the Sacrifice) must be killed in a single strike or the achievement progress will reset only for those three.
