Fragment V does not show up if you are on the campaign quest for the area, once I completed the area it showed up!

Fragment IV - /way 39.9, 21.0
is on the wooden frame, 2nd from the very top. You will likely need Steady Flight to be able to get successfully. Unless you're a dab hand at Skyriding.

I started this expansion hating Earthens and already disliking the Titans. This achievement made me start to like the Earthens and really despise the Titans.

With the release of Undermine, the zone have shifted a bit to the left due to the new area. I shifted Dathwada's coordinates by 4.2 points to make them work:
