Now that this quest has been on live servers for some hours on EU and I've completed the chapter 5 quest chain on 3 characters, I think it's time someone mentions that once you've accepted the quest in Bastion, Jaina will place a portal to Torghast in The Maw, after a few seconds of walking.
The amount of people I've seen running back out of the cave after picking up the quest is insane.

It is funny how they walk a bit (complaining about having to go back into the Maw) then Jaina eventually makes a portal.

I can totally see why so many would miss it.

Quest Name:	Start and description
1 The Broken Crown	Go to cave 33 69 and speak with Anduin Wrynn cinematic will start.
2 Our Last Option	Conversation will start
3 Hello, Darkness	Press on Shard of Domination. After that use Domination ( spell on extra bar ) on Darion Mograine. He will attack you take his HP to 70%.
4a Elder Eru	Go to Elder Eru 56 83
4b Testing One Two	Use Domination on: 3 Cervid Grazer, 3 Native Poultrid 45 71 and 2 Outmoded Servitor 57 83.
6 Cryptic Catalogue	Press on: Unknown Phrase 57 84, Unknown Phrase 58 84 and Unknown Phrase 58 81 then go inside cave 59 81
7 The Not-Scientific Method	First press on Lexicon Librarian then use on him Domination and slay him at end.
8 Two Paths to Tread	Go back to The Primus in cave 33 68.
9 One Half of the Equation	Go to see Highlord Bolvar Fordragon 57 31
10 Oppress and Destroy	Kill mobs to get to 100%. You can use Domination to kill mobs faster.
11 Aggressive Excavation	"Go to 3 points: - Inside bulding 59 31 Inscrutable Phrase - Inside bulding 62 22 Inscrutable Phrase but first press on Blocked Door then kill Agitated Geomental to get inside. - Inside bulding 65 25 Inscrutable Phrase"
12 Where the Memory Resides	Go to Bastion and speak with Highlord Bolvar Fordragon 55 86 cave enter
13 What We Wish to Forget	First press on Shard of Domination in the middle of the room and kill monsters. Press on 2nd Shard of Domination and kill mobs.
14 What Makes Us Strong	Press on Shard of Domination and kill waves of mobs xD then speak with Sylvanas Windrunner Cinematic will start
15 What We Overcome	Press on Shard of Domination and kill Mawbound Monstrosity.
16 Forge of Domination	Travel to The Runecarver's Oubliette in Torghast and speak with The Primus.
17 The Crown of Wills	Place all shards one by one. Cinematic will start after press on Crown of Wills you will get Tome of the Eternal.

To complete this quest you must have Prison of the Forgotten completed. I skipped the Torghast unlock chain and jumped straight into Zereth Mortis.

If you are skipping Torghast on an alt you will get stuck at Prison of the Forgotten. the key wont work until you complete The Highlord Calls, so you if you skipped your covenant campaign its at this point you have to go back to your covenant so you can complete up to Forge of Domination (usually only about the first 10-15 quests of the campaign just after you visit the maw and get your first soulbind). Once you complete a quest to forge with your soulbind The Highlord Calls and Forge of Domination will pop up. Tip: Although a bit tough at first by doing Zereth first as you'll get geared in 226-236 rares and epics so you can go back and breeze through covenant campaign and higher level Torghast and be appropriate ilvl for LFG Raiding.

If you're on an alt and can't get through the door because you skipped everything you could skip - go to Bolvar and choose to skip the torghast intro. After i went back and skipped the torghast intro i could enter the door.

Had to complete the covenant quest chain to the point where they sent me to Oribas to speak to Bolvar. At that point he had an option to skip the Torghas intro and the door was now open to the chamber to complete this quest.

For those looking for where the fabled "Runecarver's Oubliette Portal" might be in Torghast: it's south of where the portal dropped you. Not among all the other ones in the east.

Description

We have all of the pieces we need. The broken half of the helm has been augmented with the complexity of the language of the First Ones, and the shards bear our memories. Our will to resist Domination. All that remains is for the Primus to reforge the Helm, then we will be able to draw upon that combined power for ourselves. I know that it is dangerous, but it is our only hope.
