Eye Wish You Were Here as of 7/8/2021 Hotfix, achievement works as intended

However, in order to get this achievement you must DPS the boss down to 33% so it will trigger Immediate Extermination. There is a satchel on the back wall that is only interactable once the boss casts Immediate Extermination. This satchel will give you a single use Scavenged S.E.L.F.I.E. Camera.

In order to get this achievement you must have the raid stack so that you can fit them all in one Scavenged S.E.L.F.I.E. Camera frame. All players in the shot will recieve the Photoflash! buff.

We had the entire raid move out of frame and cast the Scavenged S.E.L.F.I.E. Camera a second time on Eye of the Jailer. Even tho it and the entire raid had Photoflash! the achievement was not granted.

This is still bugged 7/9/21 after the pet hotfix. We're still unable to get the debuff to apply to the boss, no matter what we did. We attempted the aforementioned suggestions, as well as just rotating a dozen people in the same pull to apply it with no success.

In order to take a picture of the boss you have to move a bit closer to it when you snap the picture.
You can still move while holding the camera

Do the fight as normal, bringing the eye down to 33 percent to start phase 3, during the earlier phases, make sure to keep the center of the room clear.

After the boss casts Immediate Extermination, click on the satchel in the back of the room, this contains the Scavenged S.E.L.F.I.E. Camera (note this cannot be clicked before phase 3).

Take a selfie and get everyone in the raid and the boss the Photoflash! debuff.

If you move closer to the eye makes it easier to get the boss the photoflash! debuff. (this can be a pain to do, I recommend practicing beforehand with a normal selfie camera)

Finish the fight as normal.

Did this tonight with my usual community group.

Got the boss to 33%, I grabbed the bag, and had everyone stack on a marker just in front of the boss...
I backpeddled as quick as I could towards it... and used the camera, getting everyone in the picture (and the boss) first try.

I then died 0.5secs later to something I couldn't see while holding the camera (taking 2 others with me too :P ).
However, everyone got the achy (you still have the debuff when dead)

Defeat the Eye of the Jailer after using the Scavenged S.E.L.F.I.E. Camera to take a picture of the Eye of the Jailer and the entire raid after it has cast Immediate Extermination in the Sanctum of Domination on Normal difficulty or higher.

Criteria
