I'm trying to make a macro to shift into Cat Form if I'm not in a form and not using a modifier, Bear Form if I'm not in a form and holding control, Cat Form if I'm in Bear Form, and Bear Form if I'm in Cat Form, as well as using the appropriate tooltips.
My current setup is this:
#showtooltip Cat Form; Bear Form; Cat Form; Bear Form
/cast Cat Form; Bear Form; Cat Form; Bear Form
This shows the Cat Form tooltip regardless of current form or modifier, casts Cat Form in caster form regardless of modifier, and casts Cat Form in Bear Form and vise versa, though holding control in Bear Form removes Bear Form.
Dropping causes Bear Form to show Cat Form's tooltip and vise versa, but otherwise behaves the same.
Dropping causes it to show Cat Form's tooltip and cast Cat Form regardless of form, and show Bear Form's tooltip and cast Bear Form regardless of orm when holding control.
What might I be doing wrong here?

I assume you want it to shift into bear form if in cat form without a modifier. The above will do that. Proper tooltip is preserved by conditionals. (An empty #showtooltip will choose the first ability whose conditionals evaluate true.)
As a consequence, no tooltip will be shown and no ability will be used if you are in a different form (aquatic, travel, flight). The following adjust will switch to cat form if you're in these forms and no modifier is held; and bear form if you're in these forms and ctrl is held:
#showtooltip
/use Bear Form; Cat Form
Although I suggest you rethink your setup. Having a macro that changes its behavior goes counter-intuitive to muscle memory. Why not just do what I do:
#showtooltip
/cast !Cat Form
/cast !Bear Form
/cast !Aquatic Form; !Travel Form
/cancelform
Mine also has the benefit of allowing power-shifting (eg to escape snares in PvP).

Also, if you're looking for any kind of multi-purpose shapeshifting functionality, Yaymounts has excellent Druid support plus a very workable random mount function (which is actually its primary function). I don't actually have a macro for any forms but Cat and Bear form; the rest I handle with Yaymounts.
