Herald of the Titans is the one I'm most proud of. Unlike others, you can't overgear this one, so it adds quite a nice challenge. I still use that tittle and I'm not not sure if I'll remove it when/if I get Dragonslayer, of the Four Winds or Blackwing's Bane.
I'd have to say Loremaster. It's an amazing triumph for me.

It has to be Hand of A'dal. It's a cliched answer to have one of your most prestigious achievements be your proudest, but to me I feel like it is representative of the struggles that my guild and I had in downing Vashj and Kael'thas. For both of them, it seemed like we were on them for weeks and we couldn't make much progress beyond Phase 2 of Vashj and Phase 3 of Kael. But as we smashed our heads against the encounters, we were in fact improving, and eventually it clicked and we got some very good kills (relatively smooth in the case of Vashj, down-to-the-wire and exhilarating in the case of Kael). Although Amani War Bear is rarer, that just shows how well 10 of us could execute a game plan when outgearing Zul'Aman, and Hand of A'dal felt like something more of the guild could bask in and enjoy.
Let me think... For me its my Accomplished Angler and the connected title. I love it! And I really don't know, how many fish i had to catch for it, felt like millions.
The Achievement I'm most proud of is . I got it a long time ago where it wasn't as common to have, and I was always proud of riding around my Violet Proto-Drake. Plus, I enjoyed doing all the world events and it was always something to look forward to on the calendar! I remember standing in Warsong Hold and using Mistletoe to get my -last- achievement. Was a good feeling. :D Then flying straight to Dalaran to show of my new mount!
I loved the 25man 3 drake sarth achievement because it was my first super schweet title
I would have to say my favorite achievement is Firefighter. Trying to get that during the time it was actually current was a nightmare, but definitely worth it in the end. I think that one was the hardest for the Ulduar drake metas.
I almost want to say Herald of the Titans is my favorite for getting a rare and hard title virtually no one will be obtaining again. However being able to have completed The Light of Dawn with my guild when it was current content has to be my favorite. I had been in bad guilds before, but got in a great guild that stuck together through the content and survived last tier and still are raiding today.

hands down it has to be when my very first toon got to 80. for that very first time it was just an excitement that was so awesome it made ya do a little dance lol. :D
Mine is definitely my 50 exalted reputation achievement! The only one that will be better will be my insane in the membrane when I finally finish darkmoon faire rep.
For me, it's Hand of A'dal. My guild called an unheard of 4th raid night the monday before the patch that removed the achievement went live. That night, we all came together as a guild and killed Kael, and it is still my best WoW memory. Best raid night ever.
Avest ye, Admiral! While i got it right when they released it as a feat of strength I still value it as my biggest achievement because of all the effort i went through as a holy priestess to kill all these goblins due to my stubbornness. not to mention the perks of the Admiral hat and bloodsail outfit.
