"Heroic Elite" is a higher quality of loot that drops from Heroic-mode raid boss encounters with an optional difficulty spike similar to either Algalon/Sinestra (optional hard mode-only encounter) or Firefighter/Yogg+0 (mechanic-triggered hard mode).

It is speculated that you must also complete these encounters without any difficulty adjustments enabled (similar to the Dragon Soul "Power of the Aspects" buff) for these items to drop.

Only drops when killing Protector Kaolan last. The mechanic is similar to Ulduar's Iron Council, where the highest item level drops when Steelbreaker is last to be killed.
